Microcirugía transanal endoscópica. Modalidad de tratamiento del cáncer de recto Microcirugía transanal endoscópica. Modalidad de tratamiento del cáncer de rectoIntroducción: la resección radical de recto era, hasta hace pocas décadas, el tratamiento quirúrgico estandarizado para el cáncer rectal, surgiendo en 1983 la microcirugía transanal endoscópica como nueva modalidad. Objetivo: ofrecer una actualización sobre microcirugía transanal endoscópica y describir su estado actual en el tratamiento del cáncer rectal. Material y métodos: se realizó una revisión en las bases de datos bibliográficas accesibles por Infomed, incluyendo artículos que trataran el tema en español e inglés, utilizando los descriptores de DeCS y MeSH. Desarrollo: esta técnica está indicada en tumores de recto bajo en estadio temprano, menores de 3 cm. Su uso en los T2-T3N0M0 con terapia neoadyuvante con intención curativa aún se encuentra en consenso. Ofrece mejor calidad de vida a los pacientes y mantiene buenos resultados oncológicos, con menor tiempo quirúrgico, riesgo de complicaciones y estadía hospitalaria. Conclusiones: la microcirugía transanal endoscópica es una técnica eficaz y segura para tratamiento del cáncer rectal bajo en etapas tempranas.Palabras clave: microcirugía, cirugía endoscópica, orificios naturales, neoplasias, cirugía colorrectal, enfermedades del recto.ABSTRACTIntroduction: radical resection was the gold standard in treatment of rectal cancer until few decades, when rise trans anal endoscopic microsurgery like new modality in 1983. Objective: to offer an update about trans anal endoscopy microsurgery and describe the current status of technique in treatment of rectal cancer. Material and Methods: databases reviewed using Infomed portal  was done, including articles of TEM in Spanish and English, using DeCS and MeSH’s descriptors. Development: TEM is indicated in low early rectal tumors, less than 3 centimeters. Actually there is no consensus in its use in T2-T3N0M0 with neoadyuvant therapy, like curative intent.  It offers better life quality for patients, with goods oncologic results, and fewer rates of complications, operating range and short hospital stay. Conclusions: TEM is safety and effective technique in treatment of low early rectal cancer.Keywords: Microsurgery, natural orifice, endoscopic surgery, rectal neoplasm, colorectal surgery, rectal diseases.
