A Ten-Year Study on Risk Factors of Orchiectomy in Testicular Torsion

SUMMARY

Testicular torsion is a common urological emergency among children, peaking in neonates and pubertal age with an incidence rate of 3.8 in 100.000 (0.004%) annually. This study aimed to explore the characteristics of testicular torsion cases and risk factors for orchiectomy in testicular torsion patients. This was a retrospective study on medical records of patients diagnosed with testicular torsion underwent surgery at the urology department of Dr. Hasan Sadikin General Hospital Bandung, Indonesia, from January 2009 to August 2019. Patient characteristics including age, duration of symptom, location, etiology, and degree of torsion were analyzed. A total of 86 cases of testicular torsion were identified, with most cases (54 cases, 62.8%) occured at the mean age 14.67 ± 2.4 years old. Most patients (69 patients, 80.2 %) arrived more than six hours after the onset, and 65 (75.5%) and 4 (4.6%) of them underwent orchiectomy and orchidopexy, respectively. The remaining patients (17 patients, 19.8%) came less than 6 hours after the onset and only 8 (9.3%) and 9 (10.4%) underwent orchiectomy and orchidopexy, respectively. Both duration of torsion (p = 0.000, P<0.05) and degree of torsion (p = 0.006 , P<0.05) were significantly correlated with orchiectomy. In conclusion,  the risk factor for orchiectomy is strongly related to duration and degree of torsion. The chance for testis salvation increases if torsion happens before 4.5 hours and the degree of torsion is below 180 degrees. Prompt diagnosis and treatment is the only key to testicular salvation.

Resultados: el 73,9% de los pacientes tenía más de 40 años. por cada 5 mujeres diagnosticadas con ojo seco se diagnosticó 1 hombre. El ojo rojo intermitente resultó el síntoma más frecuente y la inyección conjuntival el signo. Predominó la etiología tantálica con 44,8% y el ojo seco leve con 90,6%. El Test de Schirmer I mostró valores menores a 10 mm en 41,7% de la muestra y el tiempo de ruptura de la película lagrimal (BUT) menor de 10 segundos predominó con 60,4%. Conclusiones: predominó el ojo seco leve de etiología tantálica en mujeres mayores de 40 años, refiriendo síntomas como ojo rojo intermitente. La inyección conjuntival fue el signo fentobiomicroscópicos más característico y el Test de Schirmer y BUT fueron pruebas útiles en el diagnóstico etiológico.Palabras clave: Ojo seco, queratoconjuntivitis seca, inyección conjuntival, superficie ocular, síndrome, signos, síntomas.ABSTRACTIntroduction: the Dry Eye is defined like an illness of the tear pellicle and of the ocular surface;it has multifactorial origin that goes with ocular nuisances, visual fluctuations, unstable of the tear pellicle and potentialdamage on the ocular surface. Methods of descriptive statistic were applied for qualitative variables. Results: 73, 9% of the patients were older than 40 years old. By each 5 women diagnosed with dry eye 1 man was diagnosed. The intermittent red eye was the more frequent symptom. It prevailing the conjunctival injectionlike sign. The most frequent etiology was tantalic with 44, 8%. The light dry eye was more frequent, with 90,6%. The Schirmer´s Test showed smaller securities to 10 mm in 41,7% of the sample and the BUT smaller than 10 seconds it prevailed with 60,4%. Conclusions: the light dry eye of etiology tantalic prevailed in women older than 40 years, referring symptoms like intermittent red eye. The conjunctival injection was the most characteristic sign Phentobiomicroscopic. The Schirmer’s test and BUT were useful tests for the diagnosis of dry eye.
