Sarcoma de Kaposi: a propósito de un caso clínico, actualización del tema / Kaposi 's sarcoma: a case report, update on the topicSarcoma de Kaposi: a propósito de un caso clínico, actualización del tema / Kaposi 's sarcoma: a case report, update on the topicEl sarcoma de Kaposi (KS por sus siglas en inglés) es una enfermedad similar al cáncer. Se relaciona con la infección del VIH, y se presenta en estadios avanzados de la enfermedad, afectando el 20% de las personas con VIH que no toman medicamentos antirretrovirales. En Pinar del Río existe una baja incidencia de infección por VIH/SIDA. Dada la infrecuencia del Sarcoma de Kaposi se presenta un caso. Paciente de 50 años de edad, que después de un cuadro de dolor en epigastrio comienza con lesiones en la piel similares a hematoma en resolución, con centro más oscuro y nodular, que toman prácticamente todo el cuerpo. El paciente presenta ligera pérdida de peso, pero no prurito, no fiebre, no lesiones en mucosa oral. Se determinó Hb: 112g/l  Hto: 0.34l/l  Leucocitos: 4.4x109/l. Se encontró aumento de células mononucleares en el diferencial con linfopenia asociada; velocidad de sedimentación, colesterol, transaminasas, función renal y proteínas totales, normales, pero trombocitopenia moderada, acido úrico elevado. En el aspirado medular aparece predominio de linfocitos atípicos, con depresión relativa de las series hematopoyéticas. Gastroscopia, negativa. Biopsia de piel: Compatible con Sarcoma de Kaposi. VIH: tres exámenes durante el estudio con tiempo de intervalo entre ellos 3 meses desde el inicio resultaron negativos. Último examen viral a los 7 meses de iniciado el estudio (positivo) con Western Blot. Palabras clave: Sarcoma de kaposi, VIH. ABSTRACT Kaposi´s Sarcoma (KS) is a cancer-like disease. It is most frequent related to HIV infection, present in late stages of the disease and affecting 20 % of HIV infected people having no Highly Active Antiretroviral Therapy (HAART). Since in Pinar del Rio Province a low incidence rate of HIV/AIDS is observed , Kaposi´s Sarcoma is very infrequent. A 50 year-old male who after presenting an epigastralgia suffers from hematoma-like skin lesions, in resolution and when palpated the darker center was nodular. These lesions take practically the whole body, observing loss of weight, no fever, no itching, no oral mucosa lesions. Determining Hb: 112 g/l, Hto; 0.34/L, Leukocytes: 4.4 x 4x109/l, increase of mononuclear cells, associated to lymphopenia, but erythrocyte sedimentation rate, seric cholesterol, transaminases, kidney function and total proteins were normal. Moderated thrombocytopenia and high levels of uric acid were observed. In the bone marrow aspiration atypical lymphocytes and relative depression of the hematopoietic series prevailed. Gastroscopy was negative. Skin biopsy: Compatible with Kaposi´s Sarcoma. HIV: three exams during the study at 3 months intervals being negative from the beginning. The last viral examination carried out at 7 months after the beginning of the study resulted positive when Western Blot technique was used. Key words: Kaposi´s sarcoma, HIV.

Carcinoma de Uretra en una gestante: presentación de un caso / Urethral Carcinoma in a pregnant woman: a case reportCarcinoma de Uretra en una gestante: presentación de un caso / Urethral Carcinoma in a pregnant woman: a case reportPaciente femenina de raza blanca, de 34 años de edad y 33 semanas de embarazo, acude al Cuerpo de Guardia del Hospital Clínico Quirúrgico "Abel Santamaría Cuadrado" en Pinar del Río, con sangrado genital cuando se le realiza el examen físico; se comprueba que sangra con facilidad a través del meato uretral, donde se observa que emerge una tumoración vascularizada situada en la uretra distal. Se realizan los exámenes complementarios e imagenológicos, los cuales se encuentran en rangos normales. Se realizó el tratamiento quirúrgico exéresis de la tumoración, electrofulguración y biopsia, resultando ser un carcinoma epidermoide moderadamente diferenciado, que infiltra la submucosa que fue completamente removido y cursó con evolución clínica satisfactoria. Actualmente está asintomática. Complementary and imaging examinations were performed being in normal ranks. Surgical treatment was carried out removing the tumor, performing the electrofulguration and biopsy which resulted in a moderately differentiate epidermoid carcinoma, that infiltrated the submucosa, completely removed having the patient a clinical satisfactory progress, currently asymptomatic.Key words: carcinoma, urethra.

Caracterización clínico-epidemiológica de los ancianos hipertensos en casas de abuelos / Clinical epidemiological characterization of the hypertensive aged in the Elderly HomeCaracterización clínico-epidemiológica de los ancianos hipertensos en casas de abuelos / Clinical epidemiological characterization of the hypertensive aged in the Elderly HomeLa hipertensión arterial es la enfermedad cardiovascular más extendida universalmente; existen más de mil millones de hipertensos en el mundo. Con el objetivo de profundizar en el conocimiento de los aspectos clínico-epidemiológicos de la hipertensión en los ancianos, se realiza una investigación descriptiva, transversal y analítica. El universo de estudio lo constituyeron 130 ancianos, de ellos 77 hipertensos. Los datos fueron recogidos a través de las historias clínicas individuales, de salud familiar y una encuesta elaborada por el autor; se obtuvieron datos generales de los pacientes, así como variables para su estudio: edad, sexo, raza, factores de riesgo, compromiso de órganos diana. La muestra seleccionada fueron tres conglomerados, las Casas de Abuelos de los municipios Pinar del Río, San Juan y Martínez y Consolación del Sur. Los datos cualitativos se resumieron en frecuencias absolutas y relativas porcentuales. Se construyeron intervalos de confianza, y realizaron pruebas de hipótesis, regresión logística y pruebas no paramétricas. Se pudo apreciar que independientemente de la edad se puede presentar o no hipertensión arterial al menos en la muestra seleccionada, prevaleciendo el sexo femenino. Hubo predominio de hipertensión en la raza negra, solo el daño vascular cerebral y el color de la piel están asociados. Los factores de riesgo más frecuentes fueron el tabaquismo y la dislipidemia. ABSTRACT Hypertension is the most extended cardiovascular disease all over the World; existing more than thousand millions of hypertensive people in the World. With the purpose of deepening in the knowledge of the clinical-epidemiological hypertension in the elderly a descriptive, cross-sectional and analytic research was carried out. The universe of the study was comprised of 130 aged, out of them 77 hypertensive. Data were collected by means of the individual clinical histories, familial health and a survey designed by the author; obtaining patient's general data, and variables to the study: age, sex, race, risk factors, and influence in target organs. The sample chosen took three conglomerates, the Houses of the Elderly in Pinar del Rio, San Juan y Martinez and Consolacion del Sur. Reviewing qualitative data in absolute and relative frequencies. Confidence intervals, hypothesis testing, logistic regressions and non-parametric tests were made. Showing that regardless of age; hypertension can or cannot be present at least in the sample chosen, prevailing female sex. Observing that hypertension prevailed in black race, and that only vascular brain damage and the color of the skin had association. The most common risk factors were smoking and dyslipemia.
