High risk pregnancies and factors associated with neonatal death

SUMMARY

OBJECTIVE To identify the factors associated with intra-hospital neonatal mortality based on the individual characteristics of at-risk pregnant mothers, delivery and newborns. METHOD This was a cross-sectional epidemiological study of live newborns delivered by women attended at the high-risk outpatient unit of a philanthropic hospital in Maringá, Paraná, Brazil between September 2012 and September 2013. RESULTS Six hundred and eighty-eight women participated in the study. The neonatal mortality coefficient found was 17.7/1,000 live births, most in the early neonatal phase. Premature labor, fetal malformation and multiple gestations were associated with neonatal death. Premature, very low birth weight newborns and those with an Apgar score of less than seven, five minutes after birth were at high risk of death. CONCLUSION Identifying risk factors can help plan actions to consolidate the perinatal network. Specific programs should be incentivized in other countries, in the search for significant perinatal results such as reducing neonatal mortality.

Riesgo de diabetes mellitus tipo 2 en la población prediabética de un consultorio médico / Risk of diabetes mellitus type 2 in the population with prediabetes of a medical clinic Riesgo de diabetes mellitus tipo 2 en la población prediabética de un consultorio médico / Risk of diabetes mellitus type 2 in the population with prediabetes of a medical clinicIntroducción: la disglucemia es considerada como una condición metabólica que antecede al desarrollo de la diabetes mellitus de tipo 2.Objetivo: estratificar el riesgo de diabetes mellitus tipo 2 en pacientes prediabéticos del consultorio del médico de la familia 26 del Policlínico Universitario “Héroes del Moncada” en la ciudad de Cárdenas, provincia de Matanzas, Cuba.Métodos: se realizó un estudio observacional, descriptivo y transversal en 120 pacientes adultos diagnosticados como prediabéticos, desde enero de 2014 hasta enero de 2015. A los pacientes diagnosticados se les aplicó el Score de FINDRISC.Resultados: predominó el sexo femenino y los grupos de edades de entre 40-49 años. Los factores de riesgo más frecuentes que se identificaron fueron: obesidad y sobrepeso (38,3%), dislipidemias (19%) e hipertensión arterial (14,1%). La glucemia en ayunas alterada fue la categoría diagnóstica que aportó mayor cantidad de casos de pacientes con esta condición (53,3%). La comorbilidad asociada predominante fue la obesidad y sobrepeso. Predominó el riesgo moderado de desarrollar diabetes mellitus tipo 2, con 43 casos del total (35,8%), seguido por el grupo de riesgo ligeramente elevado con 39 pacientes (32,5%).Conclusiones: la comorbilidad asociada que más predominó fue la obesidad y sobrepeso. Se encuentra relacionada directamente con factores de riesgo cardiometabólicos. Se identificaron a varios pacientes aparentemente sanos con prediabetes y con riesgo moderado de desarrollar diabetes mellitus en los próximos 10 años.Palabras clave: prediabetes; diabetes; detección; prevención; riesgo cardiometabólico; obesidad.ABSTRACTIntroduction: the prediabetes or disglucemia are considered as a metabolic condition that proceeds to the development of the diabetes type mellitus 2.  Objective: to stratify the risk of diabetes mellitus type 2 in patient with prediabetes of the clinic of the doctor of the family 26 of the University Policlinic “Heroes del Moncada” in Cárdenas’ city, county Matanzas, Cuba. The glycemic in you fast altered the diagnostic category that contributed bigger quantity of cases with 53,3% of patient with this condition was. The predominant concomitant illnesses was the obesity and overweight. The moderate risk prevailed of developing diabetes mellitus type 2, with 43 cases representing 35,8% of the total, continued by the group of lightly high risk with 39 patients for 32,5%.Conclusions: the associate concomitant illnesses that more prevailed was the obesity and overweight. It is related directly with cardiometabolic risk factors. They were identified several seemingly healthy patients with prediabetes and with moderate risk of developing diabetes mellitus in next 10 years.Keywords: prediabetes; diabetes; screening; prevention; cardiometabolic risk factors; obesity.

Complicaciones cardiovasculares y sus factores determinantes en pacientes adultos portadores de Enfermedad Renal Crónica / Cardiovascular complications and their risk factors in adult patients with Chronic Kidney Disease Complicaciones cardiovasculares y sus factores determinantes en pacientes adultos portadores de Enfermedad Renal Crónica / Cardiovascular complications and their risk factors in adult patients with Chronic Kidney Disease Objetivo: describir las complicaciones cardiovasculares y sus principales factores determinantes en los pacientes adultos con enfermedad renal crónica del consultorio 10, Policlínico “Adrián Sansaricq” de Artemisa, desde octubre de 2016 hasta abril de 2018. Materiales y Métodos: se realizó un estudio descriptivo, longitudinal prospectivo. El universo estuvo compuesto por 856 personas. La muestra quedó representada por 214 pacientes. Las variables utilizadas fueron la clasificación de la Enfermedad Renal Crónica, causa básica, la edad, el sexo, los factores de riesgo cardiovasculares, las complicaciones, las comorbilidades y las causas de muerte. La información se obtuvo a partir de las historias clínicas individuales.Resultados: predominó la Enfermedad Renal Crónica en mujeres y adultos mayores entre 60-69 años. Prevaleció el estadio G3a en la clasificación de dicha enfermedad. Las principales causas básicas fueron la Nefropatía hipertensiva y la diabética, seguidas por la obstrucción urinaria. Se constató una alta exposición a los factores de riesgo cardiovasculares con mayor porcentaje de los fumadores y de la presencia de disturbios fosfocálcicos, mayormente en los adultos mayores. La Insuficiencia Cardíaca, las arritmias y la anemia, predominaron como comorbilidades asociadas. Falleció el 2,8% de los casos investigados, de forma predominante, por causas cardiovasculares y neoplasias.Conclusiones: la Enfermedad Renal Crónica se diagnosticó en un elevado porciento de personas, asociada principalmente a la Hipertensión Arterial, la diabetes Mellitus y los adultos mayores. ABSTRACT Objective: to describe the cardiovascular complications and their main determining factors in adult patients with chronic kidney disease in the office 10, Polyclinic “Adrián Sansaricq” of Artemis, from October 2016 to April 2018.Materials and Methods: a descriptive, longitudinal prospective study was conducted. The variables used were the classification of Chronic Kidney Disease, basic cause, age, sex, cardiovascular risk factors, complications, comorbidities and causes of death. The information was obtained from the individual medical records.Results: Chronic Kidney Disease predominated in women and older adults between 60-69 years. The G3a stage prevailed in the classification of said disease. The main basic causes were hypertensive and diabetic nephropathy, followed by urinary obstruction. There was a high exposure to cardiovascular risk factors with a higher percentage of smokers and the presence of phosphocalcic disorders, mostly in older adults. Heart Failure, arrhythmias and anemia predominated as associated comorbidities. 2.8% of the cases investigated, predominantly, due to cardiovascular causes and malignancies.Conclusions: Chronic Renal Disease was diagnosed in a high percentage of people, mainly associated with Arterial Hypertension, diabetes Mellitus and the elderly.
