Prevalence of pulmonary dysfunction in patients with beta thalassemia major: a systematic review and meta-analysis

SUMMARY

Background Many studies have been conducted on heart, liver, and endocrine abnormalities in thalassemia; however, studies on pulmonary dysfunction (PD) have been limited. Previous studies on the prevalence of restrictive lung disease (RLD) and obstructive lung disease (OLD) in β-thalassemia major patients have lacked agreement.Objective To assess the prevalence of PD in β-thalassemia major patients by systematic review of the literature and meta-analysis.Methods We searched Cochrane library, PubMed, Web of Science, MEDLINE, Scopus, and Embase for relevant articles. Articles were selected according to the inclusion criteria and data were extracted. The primary outcome was prevalence of pulmonary dysfunction in β-thalassemia major with 95% confidence interval (95%CI). Subgroup analyses were applied to explore the prevalence in different age groups, regions, and serum ferritin levels. Sensitivity analysis and publication bias assessment were also conducted.Results A total of 37 studies comprising 1,467 cases were included in this analysis. Pulmonary dysfunction was present in 64.7% (95%CI 57.6 to 71.1) of cases. The pooled prevalence of RLD (44.9%) was higher than that of OLD (7.6%) and diffusion impairment (DI) (35.6%). Subgroup analysis revealed that the region with the highest pooled prevalence of PD was the Americas (75.2%). The highest prevalence of RLD and DI was found in Asia (48.2% and 44.6%, respectively) and that of OLD in Europe (9.7%). Sensitivity analysis showed that the pooled results were robust.Conclusion A high prevalence of pulmonary dysfunction, mainly RLD rather than OLD, was detected in β-thalassemia major patients.

Clinical-epidemiological behavior of pulmonary tuberculosis in Los Palacios municipalitySe realizó un estudio descriptivo y longitudinal sobre el comportamiento clínico - pidemiológico de la Tuberculosis Pulmonar durante los años 1994 - 2000, en el municipio Los Palacios. La muestra la conforman 25 pacientes con el diagnostico positivo, que son reportados por la Unidad Municipal de Higiene y Epidemiología. De ellos, 16 pertenecen al sexo masculino (64 %) y 9 al sexo femenino (36 %). El análisis de los resultados se basó fundamentalmente en un análisis de porcientos y proporciones, estos fueron agrupados en tablas para ilustrar mejor los resultados obtenidos. Se concluye que la operación jubilado prevalece con 8 casos (32 %) seguida por /ama de casa /y /obrero/. De los 25 casos con diagnóstico positivo, prevalece la no asociación de tuberculosis con enfermedades crónicas o debilitantes, presentándose solamente 2 casos con Insuficiencia Cardíaca (8 %) y un caso asociado a Diabetes Mellitus (4 %), uno al SIDA y uno al Alcoholismo Crónico. Mas de la mitad de los casos (60 %) se diagnostican en la atención secundaria, predominando los bacilos de Kosh negativos (BK) negativos, con 19 casos (76 %). La manifestación clínica más frecuente es la tos (92 %), seguida de la expectoración (18 casos, que representan un 72 %). La prevalencia es de 6,25 por cada 10000 habitantes. Se recomienda aumentar la vigilancia epidemiológica por el medico y la enfermera de la familia. DeCS: TUBERCULOSIS PULMONAR/ epidemiología. ABSTRACT A descriptive and longitudinal study on the clinical and epidemiological behavior of pulmonar tuberculosis between 1994 - 2000 in Los Palacios Municipality is carried out. Sample consists of 25 patients with a positive diagnosis reported by the Epidemiology and Hygiene Department of the Municipality, 16 are male (64 %) and 9 are female (36 %). Results were based mainly on the percentage and ratio analysis being group in tables for a better illustration of the obtained results. It is concluded that retiree accounts of 8 cases (32 %) followed by housewives and workers. Of the total of 25 cases with positive diagnosis, tuberculosis no associated to chronic and weakening diseases was predominant, being present only two cases with heart failure (8 %) and a case associated to Diabetes Mellitus (4 %), one associated to VIH and another to chronic alcoholism. More than behalf of cases (60 %) are diagnosis in secondary care predominating negative Kosh?s bacillus, 19 cases (76 %). Cough is the most frequent clinical manifestation (92 %) followed by expectoration (18 cases, 72 %). Prevalence is 6.25 by 10000 inhabitants. Increase the epidemiological surveillance by family physician and nurse is recommended.DeCS: PULMONAR TUBERCULOSIS/ epidemiology.
