Spirometric surveillance of obstructive lung diseases

SUMMARY

Background: Obstructive Lung Disease (OLD) becomes global health issues that influencing the physical health and economic conditions of people. Chronic Respiratory Disease (CRD) is the disease of airways characterized by obstruction and influencing the structure of the lungs. It includes chronic bronchitis,COPD, emphysema and asthma. The most common disease is Chronic Obstructive Pulmonary Disease (COPD). The major causes of COPD involve exposure to an irritant that damage the lungs and airways that can cause such disease.Aim: 1): Assess the prevalence of OLD among the patients admitted to general medical wards using spirometry; 2): Use a symptom questionnaire for screening patients with OLD and compare it with Spirometry; 3): Association of various medical co-morbidities with the diagnosis of OLD.Materials and Methods: The screening for OLDs and factors that are influencing it along with co morbidities were analyzed. This is a descriptive study conducted on patients admitted to general medical wards in hospital of South India for a period of 2 years. The study has involved the patients who areadmitted in general wards and aged > 40 years.Results: The physician diagnosis of OLD at admission was in 21 out of 144 patients. It was by symptom alone in 11% (16 of 144 patients) and based on spirometry in 3.48% (5 of 144) only. During the hospitalization the diagnosis of OLD increased by 2.08% (3 of 144) on the basis of symptom alone and 1.4% (2 of 144) by spirometry, cumulatively by 3.48% (5 of 144). Therefore, at admission about 14.48% were diagnosed to have OLD. An ROC curve was plotted and the area under the curve was 0.77 95% CI (0.688 — 0.865). By ROC coordinates a cut off of 15.5 for detecting OLDs showed a sensitivity andspecificity of 71% and 78% respectively with a positive predictive value of 56% and negative predictivevalue of 87%.Conclusion: With the additional yield of Spirometry and association of medical comorbidities can help identify the conditions of patients earlier and pave way for offering appropriate holistic treatment.Keywords: Spirometric, Obstructive Lung Diseases, Pulmonary Disease.

Estrategia preventiva para la adicción al alcohol en gemelos: Pinar del Río, 2007 / A strategy for the prevention of alcoholism in twins: Pinar del Rio, 2007Estrategia preventiva para la adicción al alcohol en gemelos: Pinar del Río, 2007 / A strategy for the prevention of alcoholism in twins: Pinar del Rio, 2007Con el objetivo de proponer una estrategia de prevención para la adicción al alcohol  en gemelos, se realizó una investigación epidemiológica, analítica y transversal (estudio en gemelos) en los meses de febrero y marzo del año 2007, en la provincia de Pinar del Río. El universo fue tomado del Registro Nacional de gemelos, estudiándose el comportamiento clínico, epidemiológico y genealógico de la adicción al  alcohol en gemelos. Para la recolección de la información se confeccionó una encuesta y un árbol genealógico para cada familia. Se calculó la concordancia en gemelos, para el par y para el caso índice y la heredabilidad. Los datos se presentaron en cuadros y gráficos. Se concluyó que la mayor cantidad de los pacientes se encontraron en el grupo de mayores de 40 años de edad,  del sexo masculino, en su mayoría dependientes alcohólicos. Los antecedentes familiares de alcoholismo resultaron estadísticamente significativos en el grupo de los familiares de primer grado. La concordancia fue mayor en gemelos monocigóticos que en  dicigóticos y la contribución genética para el alcoholismo de 66%. Se confeccionó una estrategia de prevención del alcoholismo basada en los principios del asesoramiento genético. Palabras clave: alcoholismo, gemelos/genética. To collect the information a survey and a genealogical tree for each family were made. Also calculating twin's concordance for the par, case rate and inheritance; data were presented in tables and graphics; prevailing alcoholic patients older than 40 years old and male sex. Familial history of alcoholism resulted statistically significant in the group of first degree relatives. The concordance was greater in monozygotic than in dizygotic twins and genetic contribution for the alcoholism reached 66%. Evaluación de factores de riesgo en accidentes oculares graves infantiles / Assessment of the risk factors for severe ocular accidents in childrenEvaluación de factores de riesgo en accidentes oculares graves infantiles / Assessment of the risk factors for severe ocular accidents in childrenObjetivo: evaluar factores de riesgo en accidentes oculares graves infantiles. Métodos: se realizó una investigación aplicada, epidemiológica, descriptiva, longitudinal y prospectiva sobre accidentes oculares graves en el Servicio de Oftalmología del Hospital Pediátrico Provincial de Pinar del Río (2006-2007). El universo y muestra estuvo constituido por dos grupos de niños de 1- 20 años, ambos sexos y todas las etnias. Primer Grupo: Niños con accidente ocular grave, Segundo Grupo: Niños sin accidente ocular; determinándose una muestra total de 130 seleccionados mediante muestreo aleatorio simple. Fueron utilizados métodos empíricos y teóricos de investigación científica. Resultados: el trauma ocular representó el 87,8% de los ingresos de urgencia durante el período. Se encontró que el grupo etáreo de 9-12 años (40%), y sexo masculino (89,2%) fueron los más afectados. Un 76,9% de los accidentados residía en zona rural. El 96,9% se encontraba fuera de la casa y sin compañía de adultos en el momento del trauma. Predominaron las familias con conocimientos y actitudes evaluados de regulares en relación a los accidentes oculares con un 57,7 % y 60,8% respectivamente. Conclusiones: se constató elevada frecuencia de traumas oculares infantiles graves en Pinar del Río, precisando sus factores causales de riesgo y dificultades en la prevención de los mismos.Palabras clave: ACCIDENTES/clasificación, TRAUMATISMOS OCULARES, NIÑO, FACTORES DE RIESGO.ABSTRACTObjective: to assess the risk factors for severe ocular accidents in children. Methods: an applied, epidemiological, descriptive, longitudinal and prospective research of the severe ocular accidents was conducted in the Ophthalmologic Service at “Pepe Portilla” Provincial Children Hospital, Pinar del Rio. The target group and the sample were comprised of two groups of children from 1 to 16 years old of both sexes and all ethnics. First group: children and adolescents suffering from severe ocular accidents. Second group: children and adolescents without ocular accidents; a total sample of 130 patients were chosen by means of a simple sampling at random. Empiric and theoretical scientific research methods were used. Results: the ocular trauma represented 87, 8% of the admissions in the emergency service during the period. The group of ages between 9-12 and male sex (89, 2%) were the most affected; 76, 9% lived in rural zones and 96, 9% of the accidents occurred out of the house without adult company; knowledge and attitudes towards ocular accidents were evaluated as fair in families (57,7% and 60,8% respectively). Conclusions: a high frequency of severe ocular traumas in children was verified in Pinar del Rio, specifying causal risk factors and difficulties to prevent these accidents.Key words: ACCIDENTS/classification, EYE INJURIES, CHILD, RISK FACTORS.

Resultados de la capsulotomía Nd-YAG láser / Results of the capsulotomy Nd-YAG LASERResultados de la capsulotomía Nd-YAG láser / Results of the capsulotomy Nd-YAG LASERLa catarata es una de las principales causas de ceguera en todo el mundo y su extracción quirúrgica es la única terapia admitida como eficaz. Dentro de las complicaciones más frecuentes está la opacidad de la cápsula posterior del cristalino, ésta puede aparecer meses o años después de la cirugía. Ocurre entre el 20% y el 50% de los pacientes en un periodo de 5 años posteriores a la misma. La capsulotomía posterior puede realizarse mediante maniobras manuales con el instrumental tradicional, o con láser, siendo este último la modalidad habitual en la época actual; por estas razones se realiza esta investigación, para conocer los resultados obtenidos después de la capsulotomía con láser en los pacientes operados de catarata por la Técnica Blumenthal en el Hospital Universitario Abel Santamaría Cuadrado, en Pinar del Río (Cuba), durante el periodo junio 2007-mayo 2008. Para ello se tuvieron en cuenta las variables: edad, sexo, implante o no de lente intraocular, complicaciones trans y post láser, así como agudeza visual pre y post tratamiento. El 80.2% de los pacientes estudiados tenía >60 años de edad y en un 96,8% se colocó lente intraocular en cámara posterior. El 65.9% de los ojos presentó elevación transitoria de la presión intraocular y sólo en un 3.2% hubo astillamiento del lente intraocular sin necesidad de extracción. Las complicaciones no cobraron importancia si se considera que el 93.2% de los ojos alcanzó una agudeza visual >0.7. Palabras clave: catarata/complicaciones, pacientes.ABSTRACT Cataracts are one of the main causes of blindness all over the world and its surgical removal is the only effective therapy admitted. The opacity of the posterior capsule of the lens is the most frequent post-surgical complication. This may appear months or years after surgery and is present in 20-50 % of the patients after 5 years of their surgical intervention. The posterior capsulotomy may be performed by means of manual procedures using the traditional instrumentation, or by means of laser; the current technique nowadays. Showing the results after laser capsulotomy in patients operated by the Blumenthal Technique (2007-2008) at "Abel Santamaria Cuadrado" University Hospital, in Pinar del Rio Cuba was the purpose of this paper. Age, sex, implant and non- implant of the intraocular lenses (IOL), complications during trans and post laser therapy, as well as the visual acuity before and after treatment were the variables considered, all patients were older than 60 years (80, 2 %) and in 96, 8 % an intraocular lens was placed into the posterior chamber, 65.9 % of the eyes showed transient increasing of the intraocular pressure, and only 3, 2 % showed splintering of the IOL, but needing no extraction. Complications were not so important if it is considered that 93, 2 % of the eyes reached to a visual acuity > 0, 7. Key words: cataractc/complications, patients.
