Social Medicine Grand Rounds: A Collaborative Education Hub to Promote Action on Social Determinants of Health Across Community Sectors in Rural Alaska

SUMMARY

Background: The connection between social stratification and health inequalities in the United States suggests the need for medical education and practice to address social determinants of health as a key condition for achieving health equity. This reading of the project of social medicine—to wed health care delivery to a pragmatic and operationalized equity agenda—is particularly relevant to tribal health systems, where patient populations bear a disparate burden of injury and disease, where social and historical forces play a large role in the generation and maintenance of these health disparities, and where social and economic forces constrain the availability, accessibility, and efficacy of health care. Objectives: This paper describes the conceptual framework and educational materials from a social medicine grand rounds program in an Alaskan arctic tribal health system, aimed at building clinical, organizational, and community capacity to address social determinants of health. Led by the Maniilaq Association Division of Social Medicine with partners at Harvard Medical School and Massachusetts General Hospital, social medicine grand rounds bring together hospital and village clinic-based care teams, social and tribal service workers, and community members to study and apply social medicine perspectives to health care delivery across 12 circumpolar Alaska Native villages. The model utilizes a case-based curriculum to drive clinical-community collaboration and critical analysis of health care delivery challenges focused on four broad priority areas established by tribal leadership: maternal and child health, chronic disease, mental health, and infectious disease.Methods: The social medicine grand rounds program was piloted in April 2017 and will be formally launched in November 2017. Researchers will utilize a mixed methods approach, pairing analysis of electronic health record data with participant interviews and surveys to investigate changes in provider practice patterns relating to action on social determinants of health, community and cross-sector partnerships, and revisions to health system policies and protocols over one year of programming. Pre-mid-post surveys collected before the program begins, at the six-month mark, and after a year of programming will assess participants’ knowledge, readiness, and action to address social determinants of health. A social needs assessment protocol administered in the outpatient clinic will evaluate patients’ met and unmet needs, and a discrete range of health outcomes will be tracked using patient registries in the electronic health record.Discussion: We hypothesize that by maneuvering social determinants of health into the sphere of moral concern of health workers; by underscoring the clinical relevance and utility of social theory and analysis; by strengthening community partnerships linking primary care to social and tribal services; and by building a practical skill set among health workers to include social and structural interventions as part of the core clinical repertoire, social medicine education may help to mitigate the impacts of social stratification on health outcomes in Northwest Alaska. Ultimately, such projects can play a minor role in redressing structures of inequality that both produce and are propagated by poor health. Conclusion: Social medicine grand rounds examine how social, historical, and structural forces are embodied as illness and injury in individuals, as well as how these forces shape medical efficacy, illness experience, and standard of care. The model serves to build inter-professional communities of practice for learning, deliberation, and action on social determinants of health in Northwest Alaska, and to build and leverage shared clinical, education, and training infrastructure with academic partners to expand the reach of the program and reduce health inequities. Las Cátedras Multidisciplinarias y Honoríficas de la Escuela Latinoamericana de Medicina: una vía para la formación de recursos humanos / The Multidisciplinary and Honorary Chairs of the Latin American School of Medicine: a way to train human resources Las Cátedras Multidisciplinarias y Honoríficas de la Escuela Latinoamericana de Medicina: una vía para la formación de recursos humanos / The Multidisciplinary and Honorary Chairs of the Latin American School of Medicine: a way to train human resourcesObjetivo: fundamentar los logros obtenidos por las Cátedras Multidisciplinarias y Honoríficas de la Escuela Latinoamericana de Medicina en la formación de recursos humanos.Métodos: se realizó una investigación descriptiva que estudió la población y sus necesidades, se sustentó en el paradigma materialista dialéctico y se aplicaron los métodos teóricos, empíricos y estadísticos para la obtención, el procesamiento y el análisis de los resultados.Resultados: La participación en eventos, la realización de seis talleres provinciales dedicados al desempeño de las cátedras en la escuela y el aumento de los miembros en cada una de ellas constituyeron algunos de sus principales resultados.Conclusiones: el trabajo de las Cátedras Honoríficas y Multidisciplinarias en la Escuela Latinoamericana de Medicina está en correspondencia con las exigencias actuales de la sociedad cubana y contribuye a la formación integral de profesores, trabajadores y estudiantes que la integran, potenciando su participación en el funcionamiento de actividades, con el empleo del tiempo en una superación académica, cultural y política de alto valor social.The Multidisciplinary and Honorary Chairs of the Latin American School of Medicine: a way to train human resourcesABSTRACTObjective: to base the achievements obtained by the Multidisciplinary and Honorific Chairs of the Latin American School of Medicine in the training of human resources.Methods: a descriptive investigation was carried out that studied the population and its needs, it was based on the dialectical materialist paradigm and the theoretical, empirical and statistical methods were applied to obtain, process and analyze the results.Results: The participation in events, the realization of six provincial workshops dedicated to the performance of the chairs in the school and the increase of the members in each one of them constituted some of their main results.Conclusions: the work of the Honorary and Multidisciplinary Chairs in the Latin American School of Medicine is in correspondence with the current demands of Cuban society and contributes to the integral formation of professors,workers and students that integrate it, enhancing their participation in the functioning of activities, with the use of time in an academic, cultural and political improvement of high social value.

The Historical-Logical analysis, systematization, modeling, system approach.Results: it was possible to scientifically argue the importance of developing a pedagogical strategy proposal for improving the performance of tutors in education at work at the Latin American School of Medicine, contributing to improve the scientific production of our Institution.Conclusions: it is stated that the activity of tutors, is a key element for the achievement of the teaching-learning process in education at work, is the most important actor in the system of educational influences that act in the process of training the professional personality of the General Basic Physician and that transcends the strictly academic, reaching the personal and social sphere of the young person for what constitutes a fundamental link in the formativeprocess.

Cancer Cérvico Uterino en la Tercera Edad: P. del Río, 2005-2008 / Cervical Cancer in the aged: Pinar del Río, 2005-2008Cancer Cérvico Uterino en la Tercera Edad: P. del Río, 2005-2008 / Cervical Cancer in the aged: Pinar del Río, 2005-2008El propósito del presente trabajo ha sido caracterizar epidemiológica y clínicamente al cáncer cérvicouterino en la tercera edad en Pinar del Río. Método: Diseño observacional, descriptivo, transversal. Universo: Mujeres de 60 años y más de la región oeste de la Provincia Pinar del Río que tuvieron biopsias que interesaban el cuello uterino. Variables: edad, motivo de consulta, diagnóstico, rendimiento citológico, incidencia y mortalidad (por 100000 mujeres de ese grupo de edades) y letalidad en %. Resultados: Los motivos principales de consulta fueron la lesión macroscópica y el sangrado posmenopáusico. Predominó el subgrupo de 60-64 años (rango 60-94). Casi el 50% tenía lesiones cervicales. La citología mostró una sensibilidad de 9,8%, especificidad de 96,5%, valor predictivo positivo de 71.4% y el negativo de 54.7%. El carcinoma escamoso invasor (CINV) en cualquiera de sus estadios presentó una incidencia promedio anual de 57,4; el adenocarcinoma endocervical (AEC) de 8,0 y el adenocarcinoma endometrial (AEM), 35.6. No hubo diferencias de edad (F=0.52; p = 0.76). El carcinoma escamoso in situ tuvo una incidencia anual 10,2. La mortalidad anual por CINV fue de 14,3 y por AEC, 3,6, mientras que por AEM fue de 11,3. La letalidad mayor se observó en el AEC (54.5 %), Conclusiones. Las mujeres de la tercera edad aportan lesiones malignas con altas tasas de mortalidad y letalidad por lo que deben ser priorizadas ante cualquier signo macroscópico o sangrado posmenopáusico. Debe hacerse un seguimiento más continuo en la mujer antes de arribar a 60 años para evitar la morbilidad y la muerte. The medical literatures browsed on line were published on: Cochrane, Dynamed, Evidence-based Medicine Updates, New England Journal of Medicine, J Clinical Oncology, Medscape, PubMed, and the International Agency of Cancer in France. The following aspects were revised: definitions, epidemiology, etiology, risk factors, cytology, colposcopy, histopathology, classification, clinical stages, treatments, prevention, health promotion and social problems. Cervical Cancer is a preventable and curable disease, which is sexually transmitted by Human Papillomavirus having an annual higher incidence and prevalence; despite the screening programs it is still a health problem. Alternatives of screening are needed and they should be adequate to the local necessities, establishing a sustainable prevention and treatment to diminish the social and economic burden derived from this problem.
