Causes and Risk Factors of Hospitalization among Under-five Children in Kassala, Eastern Sudan

SUMMARY

BACKGROUND: According to the World Health Organization estimation, African Region deaths of all under-five deaths in 2015 were over 5 times higher in comparison to the European Region.AIM: The study aimed to estimate the prevalence, investigate the possible causes and risk factors associated with under-five children’s hospitalization in Kassala, Eastern Sudan.METHODS: A community-based cross-sectional study was conducted from December 2015 to March 2016. The data were collected by interviewing mothers.RESULTS: A total of 297 mother-child pairs participated in the study. The mean (SD) of maternal age and children’s age was 27.6 (5.9) years and 16 (11.3) months, respectively. One hundred and three children were hospitalized over the past 6 months. The most common mentioned causes for the last hospitalization were gastroenteritis 28.1% (29/103), respiratory tract infections 19.4% (20/103), malaria 9.7% (10/103), and trauma 3.8% (3/103). In multivariable analysis, a high birth order (adjusted odds ratio [AOR] 1.25, 95% Confidence Interval [CI] (1.06, 1.47), low paternal education (AOR 2.89, 95% 1.32, 6.30), and bottle feeding (AOR 2.26, 95% CI 1.30, 3.80) were associated with under-five children’s hospitalization.CONCLUSIONS: More than one-third of the children were hospitalized in Eastern Sudan. Urgent action is required to address children’s health issues (i.e., the above-mentioned causes and associated factors).

Morbimortalidad materna / Maternal morbimortalidad Morbimortalidad materna / Maternal morbimortalidadObjetivo: abordar la morbimortalidad materna teniendo en cuenta las causas y las consecuencias que trae para el recién nacido y para la madre.Métodos: se realizó una revisión bibliográfica, se utilizaron 34 referencias bibliográficas.Resultados: la mortalidad materna como indicador expresa el riesgo de morir durante el embarazo o dentro de los 42 días siguientes, por complicaciones obstétricas de la gestación, antes del embarazo o durante la gestación que no es producto de causas obstétricas directas, pero sí agravadas por los efectos del embarazo, la actualización,la capacitación del personal vinculado a la atención materna, la auditoría médica de los servicios obstétricos y el  análisis de la morbilidad materna extremadamente grave, permiten analizar los casos obstétricos complicados paraencontrar los factores relacionados y buscar soluciones que minimicen el riesgo para brindar una mejor atención a estas pacientes.Conclusiones: la actualización y la capacitación continua del personal vinculado a la atención materna, la auditoría médica de los servicios obstétricos y el análisis de la morbilidad materna extremadamente grave en cada institución,permiten analizar los casos obstétricos complicados para encontrar los factores relacionados y buscar soluciones que minimicen el riesgo y conduzcan a brindar una mejor atención a estas pacientes.ABSTRACTObjective: to address maternal morbidity and mortality taking into account the causes and consequences it brings to the newborn and to the mother.Methods: a bibliographic review was carried out; 34 bibliographic references were used.Results: maternal mortality as an indicator expresses the risk of dying during pregnancy or within 42 days,due to obstetric complications of pregnancy, before pregnancy or during pregnancy that is not the productof direct obstetric causes, but is aggravated by the effects of pregnancy, updating, the training of personnel linked to maternal care, the medical audit of obstetric services and the analysis of extremely severe maternal morbidity, allow us to analyze complicated obstetric cases to find the related factors and look for solutions that Minimize the risk to provide better care to these patients.Conclusions: the updating and continuous training of personnel linked to maternal care, the medical auditof obstetric services and the analysis of extremely serious maternal morbidity in each institution, allow us to analyze complicated obstetric cases to find the related factors and find solutions that minimize the risk and lead to provide better care to these patients.Keywords: maternal mortality; risk factor’s; prevention.

Analysis of six years of the municipality SandinoIntroducción: En los últimos años ha aumentado la importancia de las anomalías congénitas y enfermedades hereditarias como causa de muerte en el primer año de vida. El Programa de Diagnóstico y Prevención de Enfermedades Genéticas ha contribuido a la reducción de las tasas de mortalidad infantil, los indicadores de morbilidad mejorar la calidad de vida en la comunidad. Objetivos: Realizar el análisis del comportamiento del Programa de genética médica en el municipio Sandino durante seis años de funcionamiento. Método: Se realizó un estudio descriptivo, prospectivo, de corte longitudinal del total de gestantes desde el 1 de enero de 2000 hasta el 31 de diciembre de 2005, en las dos Áreas de Salud del municipio utilizando datos de la consulta municipal y provincial de Genética. Resultados: 2388 gestantes asistieron a Consulta Genética Comunitaria. 725 de ellas (30.4%) se clasificaron con riesgo incrementado. Los principales fueron la edad materna avanzada, embarazo en la adolescencia y los antecedentes familiares. El 9.84% de las Alfafetoproteínas resultaron elevadas, 48.9 % sin causa explicable. 11 pacientes decidieron no realizarse el diagnóstico prenatal citogenético y de los realizados 1 fue positivo. Los 31 casos de malformaciones detectadas decidieron interrumpir el embarazo. Conclusiones: El acercamiento de los Servicios de Genética Médica a la población y el desarrollo de la Genética Comunitaria facilitan la detección de riesgos si los casos son remitidos a tiempo al servicio de Genética. El embarazo en edades extremas conlleva los principales riesgos genéticos presentes en las gestantes estudiadas. Por cada fallecido se realizaron 6,2 interrupciones de embarazos por malformaciones. Palabras clave: Mortalidad infantil. anomalía congénita, enfermedad hereditaria, diagnóstico prenatal.ABSTRACT Introduction: In the last years Congenital Anomalies and hereditary diseases have gained importance, as a cause of death during the first year of life. The program for the ¨Diagnosis and Prevention of Genetic Diseases¨ has contributed to the reduction of infant mortality rates, and the morbility indicators, and to improve the quality of life within the community. Objective: To analyze how well the Genetic Program has been functioning in Sandino during the last six years. Method: It was held to descriptive, prospective study of longitudinal out with 100% of all pregnant women from January 1st, 2000 up to December 31st, 2005, in two health areas of the municipality, using data found in both the municipal and the provincial community Genetics Medical Assistance. Results: 2,388 pregnant women were attended in the community's Genetics Assistance, out of them just 725 (30, 4) were detected with increased risk. The main causes were advanced maternal age, teenage pregnancy and family records. Around 9.84% Alpha-fetoproteins reached high levels, 48.9% without a reasonable cause. Eleven patients decided not to go though the cytogenetic prenatal diagnosis and one resulted positive out of those carried out. The 31 cases of malformations detected decided to interrupt their pregnancy. Conclusion: The approach of the Genetic Service in the population y the Community's Genetics assistance contribute to the detection of risks as long as they go to visit the Genetic Physician on time. Pregnancy in deadline age constitutes the main genetic risk present in the pregnant women studied. For each person deceased about 6.2 abortions of pregnancy were done because of malformations. Complicaciones cardiovasculares y sus factores determinantes en pacientes adultos portadores de Enfermedad Renal Crónica / Cardiovascular complications and their risk factors in adult patients with Chronic Kidney Disease Complicaciones cardiovasculares y sus factores determinantes en pacientes adultos portadores de Enfermedad Renal Crónica / Cardiovascular complications and their risk factors in adult patients with Chronic Kidney Disease Objetivo: describir las complicaciones cardiovasculares y sus principales factores determinantes en los pacientes adultos con enfermedad renal crónica del consultorio 10, Policlínico “Adrián Sansaricq” de Artemisa, desde octubre de 2016 hasta abril de 2018. Materiales y Métodos: se realizó un estudio descriptivo, longitudinal prospectivo. El universo estuvo compuesto por 856 personas. La muestra quedó representada por 214 pacientes. Las variables utilizadas fueron la clasificación de la Enfermedad Renal Crónica, causa básica, la edad, el sexo, los factores de riesgo cardiovasculares, las complicaciones, las comorbilidades y las causas de muerte. La información se obtuvo a partir de las historias clínicas individuales.Resultados: predominó la Enfermedad Renal Crónica en mujeres y adultos mayores entre 60-69 años. Prevaleció el estadio G3a en la clasificación de dicha enfermedad. Las principales causas básicas fueron la Nefropatía hipertensiva y la diabética, seguidas por la obstrucción urinaria. Se constató una alta exposición a los factores de riesgo cardiovasculares con mayor porcentaje de los fumadores y de la presencia de disturbios fosfocálcicos, mayormente en los adultos mayores. La Insuficiencia Cardíaca, las arritmias y la anemia, predominaron como comorbilidades asociadas. Falleció el 2,8% de los casos investigados, de forma predominante, por causas cardiovasculares y neoplasias.Conclusiones: la Enfermedad Renal Crónica se diagnosticó en un elevado porciento de personas, asociada principalmente a la Hipertensión Arterial, la diabetes Mellitus y los adultos mayores. ABSTRACT Objective: to describe the cardiovascular complications and their main determining factors in adult patients with chronic kidney disease in the office 10, Polyclinic “Adrián Sansaricq” of Artemis, from October 2016 to April 2018.Materials and Methods: a descriptive, longitudinal prospective study was conducted. The variables used were the classification of Chronic Kidney Disease, basic cause, age, sex, cardiovascular risk factors, complications, comorbidities and causes of death. The information was obtained from the individual medical records.Results: Chronic Kidney Disease predominated in women and older adults between 60-69 years. The G3a stage prevailed in the classification of said disease. The main basic causes were hypertensive and diabetic nephropathy, followed by urinary obstruction. There was a high exposure to cardiovascular risk factors with a higher percentage of smokers and the presence of phosphocalcic disorders, mostly in older adults. Heart Failure, arrhythmias and anemia predominated as associated comorbidities. 2.8% of the cases investigated, predominantly, due to cardiovascular causes and malignancies.Conclusions: Chronic Renal Disease was diagnosed in a high percentage of people, mainly associated with Arterial Hypertension, diabetes Mellitus and the elderly.
