Has the increase in the availability of generic drugs lowered the price of cardiovascular drugs in South Africa?

SUMMARY

AbstractBackground: This research focuses on pharmaceutical competition in South Africa where concurrent pricing legislation is being implemented without monitoring the consequences on generic drug competition and usage.Objective: To examine the relationship between originator drug prices and the number of generic brands within the cardiovascular class of drugs and to compare South African prices with international reference prices.Method: Data on private sector drug prices was sourced from the South African Medicine Price Registry. The relationship between the median proportional price and the number of brands in the therapeutic class was analysed using correlation analysis. International reference prices were obtained from the Management Sciences for Health International Drug Price Indicator Guide (2012 edition).Results: A weak correlation between originator and generic drug prices and the number of available brands was observed, the exception being diuretic drugs. The median prices per strength of the originator generic were still higher than the most expensive generic version manufactured by any other company, the exception being telmisartan. Comparison of price ratios between the originator drug, lowest priced generic and international reference price values revealed that the originator drug prices had a median price ratio of 20.99 (interquartile range 7.31—53.46) and the lowest priced generics had a median price ratio of 4.28 (interquartile range 2.10—8.47).Conclusion: Increased generic competition is not a predictor of lower drug prices. The study also concludes that the current South African pharmaceutical policies have not yet achieved the lowest prices for drugs when compared internationally.

Uso inapropiado de antibióticos en pacientes cubanos hospitalizados con virus de inmunodeficiencia humana / Inappropriate use of antibiotics in Cuban patients hospitalized with human immunodeficiency virus Uso inapropiado de antibióticos en pacientes cubanos hospitalizados con virus de inmunodeficiencia humana / Inappropriate use of antibiotics in Cuban patients hospitalized with human immunodeficiency virusObjetivo: pacientes con infección por virus de inmunodeficiencia humana hospitalizados en el Instituto “Pedro Kourí” en el período desde septiembre hasta diciembre de 2016.Material y Método: estudio de corte transversal con componente analítico. El universo estuvo constituido por los 99 pacientes ingresados en el centro hospitalario del Instituto Pedro Kourí (IPK) en el período desde septiembre hasta diciembre de 2016 con infección por el virus de inmunodeficiencia humana a los que se le administró antibioticoterapia sistémica durante más de 48 horas.Resultados: se identificó uso inapropiado de antimicrobianos en el 40,4% de los pacientes. Las principales indicaciones inapropiadas fueron no ajustarse a los protocolos hospitalarios (37,5%) y el uso de los antibióticos con espectro innecesariamente amplio (37,5%). Esto fue más evidente en las neumonías bacterianas de los pacientes con mal uso de los antibióticos (40,4%). Se halló una asociación significativa entre el mal uso de los antibióticos y las estadías hospitalarias prolongadas, así como entre la inmunosupresión avanzada y el uso inapropiado de estos fármacos.Conclusiones: es frecuente el uso inapropiado de los antimicrobianos en los pacientes con el virus de inmunodeficiencia adquirido, de manera particular en las neumonías bacterianas. Esto parece estar favorecido por la condición de los inmunodeprimidos, vulnerando el ajuste a los protocolos hospitalarios establecidos. El empleo deficiente de los antimicrobianos a su vez repercute en la prolongación de estadías hospitalarias.Palabras clave: antibióticos; resistencia bacteriana; enfermedades infecciosas.ABSTRACTObjective: to characterize the use of antibiotics in patients with human immunodeficiency virus infection hospitalized at the “Pedro Kourí” Institute in the period from September to December 2016.Material and Method: cross-sectional study with analytical component. The universe consisted of the 99 patients admitted to the hospital of the Pedro Kourí Institute (IPK) in the period from September to December 2016 with infection by the human immunodeficiency virus who was administered systemic antibiotic therapy for more than 48 hours.Results: inappropriate use of antimicrobials was identified in 40.4% of patients. The main inappropriate indications were not to comply with hospital protocols (37.5%) and the use of antibiotics with unnecessarily broad spectrum (37.5%). This was most evident in bacterial pneumonia of patients with antibiotic misuse (40.4%). A significant association was found between the misuse of antibiotics and prolonged hospital stays, as well as between advanced immunosuppression and inappropriate use of these drugs.Conclusions: inappropriate use of antimicrobials is common in patients with acquired immunodeficiency virus, particularly in bacterial pneumonias. This seems to be favored by the condition of the immunocompromised, violating the adjustment to established hospital protocols. Poor use of antimicrobials in turn affects the prolongation of hospital stays.Keywords: antibiotics; infectious diseases; bacterial resistance.
