Management Comprehensive Multidisciplinary of Malignant Ovarian Germ Cell Tumors and Feto - Maternal Outcome: A Case Series Report and Literature Review

SUMMARY

BACKGROUND: Malignant Ovarian Germ Cell Tumors (MOGCT) most commonly occur in young women in the reproductive age group. Timely antenatal diagnosis and treatment of the tumour to enhance maternal and perinatal outcomes are the main challenges confronting the obstetrician and the gyne-oncologist.CASE PRESENTATION: Here we present three cases of pregnancy complicated with MOGCTs. The first case (immature teratoma) was complicated by maternal psychological symptoms consistent with stress and histopathological examination confirmed the diagnosis of premature ovarian failure (POF). The second case (dysgerminoma) preterm labour occurred as an obstetric complication, but the baby was born in good condition without IUGR. The third case (yolk sac tumour) treated with docetaxel (brexel)-carboplatin chemotherapy administration there was no maternal or fetal complication. At the end of the pregnancy and delivery, complete surgical staging and cytoreduction were performed, and no metastases were found.CONCLUSION: Optimal management strategies centre on a multi-disciplinary comprehensive team approach is critical resulting in better outcomes for the mother and the baby by avoiding complications.

Se utilizaron un total de 45 referenciasbibliográficas pertinentes para el desarrollo de esta investigación.Los queloides son neoplasias benignas del tejido conectivocompuestas de masas densas de colágeno. Su etiología es variabley los tratamientos se direccionan a la exéresis del tejido afectadoy control de la respuesta fibroproliferativa. Los tratamientosaceptados varían desde el manejo médico hasta la resecciónquirúrgica y la combinación de ambos.Conclusiones: el tratamiento de los queloides es un reto para loscirujanos dada su alta recurrencia. Keloidsare benign connective tissue neoplasms composed of densecollagen masses. Its etiology is variable and the treatments aredirected to the removal of the affected tissue and control of thefibroproliferative response. The accepted treatments vary frommedical management to surgical resection and the combinationof both.Conclusions: treating keloid scars is a challenge for surgeonsgiven its high recurrence. Although therapeutic treatments are welldocumented and widely known, it is necessary to use jointly andsuccessively multiple therapies such as surgery, drugs infiltration,pressotherapy, among others to guarantee a better result.Keywords: pathological scar; keloid scar, cicatrix, granulationtissue.

HOSPITAL FELIX MAYORIntroducción: Actualmente, el cuidado integral de los pacientes que sufren traumatismos constituye un desafío de vital importancia, sobre todo por parte del personal de enfermería, encargado de brindar los cuidados paliativos necesarios que garanticen el control del dolor y de otros síntomas y disrupciones emocionales que pueden desencadenarse enpersonas aquejadas por estas afecciones.Objetivo: Analizar la influencia de los cuidados paliativos de enfermería y el manejo del dolor irruptivo incidental en pacientes con trauma del Hospital Félix Mayorca Soto, Tarma – Perú.Resultados: el 45,5% de profesionales de enfermería maneja eficazmente el dolor irruptivo incidental en pacientes con trauma; el 2,3% desarrolla cuidados paliativos muy favorables y el 43,2% de modo favorable; mientras que el 36,4% logra un control poco eficaz del dolor en los pacientes atendidos.AbstractIntroduction: Currently, the comprehensive care of patients who suffer trauma constitutes a challenge of vital importance, especially on the part of the nursing staff, in charge of providing the necessary palliative care that guarantees the control of pain and other symptoms and emotional disruptions that They can be triggered in people with these conditions.Objective: To analyze the influence of palliative nursing care and the management of incidental breakthrough pain in patients with trauma at the Hospital Félix Mayorca Soto, Tarma-Peru.Results: 45.5% of nursing professionals effectively manage incidental breakthrough pain in trauma patients; 2.3% developed very favorable palliative care and 43.2% favorably; while 36.4% achieved ineffective pain control in the patients attended.Conclusions: incidental breakthrough pain is frequent in patients with trauma, which has a negative impact on their health prognosis by influencing the worsening of symptoms and prolonging recovery. Palliative nursing care with a comprehensive approach favorably influences compliance with pharmacological treatment and pain monitoring and control, achieving greater well-being and quality of life in these patients.
