When to use and not to use spirometry in patients admitted to the internal medicine wards for the diagnosis of chronic obstructive pulmonary disease

SUMMARY

Chronic obstructive pulmonary disease (COPD) is one of the main causes for admission to an internal medicine ward, due to exacerbations of the disease itself or to comorbidities leading to dyspnea as an intriguing symptom. In many cases, a diagnosis of COPD is made only on clinical grounds, but well-accepted guidelines strongly suggest measuring the lung function to diagnose and stage such a common disease. Problems with the equipment, its use and data interpretation lead to underuse spirometry in general and in patients in internal medicine in particular.

El universo de estudio estuvo constituido por la totalidad de las mujeres residentes en el área de salud Hermanos Cruz. Se seleccionó un grupo de estudio conformado por las mujeres obesas mayores de 50 años y un grupo de control conformado por las mujeres mayores de 50 años no obesas. Para obtener los datos, se utilizó una encuesta-formulario, donde se incluyeron variables generales de la mujer, antecedentes personales y familiares e historia ginecobstétrica, la cual fue aplicada a ambos grupos, con previo consentimiento. Se realizó un estudio ultrasonográfico a las mujeres incluidas en la investigación y uno histológico del endometrio cuando el examen ultrasonográfico resultó anormal (mayor de 4 mm en posmenopáusicas y de 14 mm en premenopáusicas.). Se confeccionó una base de datos con las variables estudiadas. Los resultados obtenidos se agruparon en cuadros (simples y de contingencia). Para su procesamiento estadístico se utilizaron la frecuencia absoluta y porcentual, el estadígrafo Ji cuadrado, pruebas de probabilidad de Fisher, los odds ratio (crudo y ponderado) y Z. La obesidad estuvo asociada con mayor probabilidad de tener un resultado ultrasonográfico anormal de la medida del grosor endometrial y de lesiones premalignas y cáncer endometrial, principalmente cuando existieron antecedentes familiares de cáncer de mama o colon y antecedentes personales de menarquía precoz, paridad y sangrado uterino anormal. ABSTRACT A retrospective and analytic research was carried out with the purpose of identifying risk factors associated with endometrial lesions in obese women older than 50 years old in Pinar del Rio municipality. The universe was comprised of the total of women living at "Hermanos Cruz" health sector. A group of obese women older than 50 years old and a control group of non-obese women older than 50 years old were chosen. To collect data, a survey-form was used; including general variables of women, personal, familial, gynecological and obstetric records. The survey was applied for both groups having a previous consent. A sonographic study was conducted to women participating in the research and performing an endometrial-histological examination when the Sonography showed abnormalities (greater 4mm in postmenopausal and 14mm in premenopausal women). A database using the variables studied was created; and the results were grouped in charts (simple and of contingency). In the statistical process absolute frequency and percentage, chi square, Fisher's probability test, odds ratio (raw and adjusted), and Z test were used. Obesity was associated with a major probability of having an abnormal sonographic result of the endometrial thickness, premalignant lesions and endometrial cancer; mainly when familial history of breast or colon cancers existed as well as personal records of early menarche, parity and abnormal uterine bleeding. Sarcoma de Kaposi: a propósito de un caso clínico, actualización del tema / Kaposi 's sarcoma: a case report, update on the topicSarcoma de Kaposi: a propósito de un caso clínico, actualización del tema / Kaposi 's sarcoma: a case report, update on the topicEl sarcoma de Kaposi (KS por sus siglas en inglés) es una enfermedad similar al cáncer. Se relaciona con la infección del VIH, y se presenta en estadios avanzados de la enfermedad, afectando el 20% de las personas con VIH que no toman medicamentos antirretrovirales. En Pinar del Río existe una baja incidencia de infección por VIH/SIDA. Dada la infrecuencia del Sarcoma de Kaposi se presenta un caso. Paciente de 50 años de edad, que después de un cuadro de dolor en epigastrio comienza con lesiones en la piel similares a hematoma en resolución, con centro más oscuro y nodular, que toman prácticamente todo el cuerpo. El paciente presenta ligera pérdida de peso, pero no prurito, no fiebre, no lesiones en mucosa oral. Se determinó Hb: 112g/l  Hto: 0.34l/l  Leucocitos: 4.4x109/l. Se encontró aumento de células mononucleares en el diferencial con linfopenia asociada; velocidad de sedimentación, colesterol, transaminasas, función renal y proteínas totales, normales, pero trombocitopenia moderada, acido úrico elevado. En el aspirado medular aparece predominio de linfocitos atípicos, con depresión relativa de las series hematopoyéticas. Gastroscopia, negativa. Biopsia de piel: Compatible con Sarcoma de Kaposi. VIH: tres exámenes durante el estudio con tiempo de intervalo entre ellos 3 meses desde el inicio resultaron negativos. Último examen viral a los 7 meses de iniciado el estudio (positivo) con Western Blot. Palabras clave: Sarcoma de kaposi, VIH. ABSTRACT Kaposi´s Sarcoma (KS) is a cancer-like disease. It is most frequent related to HIV infection, present in late stages of the disease and affecting 20 % of HIV infected people having no Highly Active Antiretroviral Therapy (HAART). Since in Pinar del Rio Province a low incidence rate of HIV/AIDS is observed , Kaposi´s Sarcoma is very infrequent. A 50 year-old male who after presenting an epigastralgia suffers from hematoma-like skin lesions, in resolution and when palpated the darker center was nodular. These lesions take practically the whole body, observing loss of weight, no fever, no itching, no oral mucosa lesions. Determining Hb: 112 g/l, Hto; 0.34/L, Leukocytes: 4.4 x 4x109/l, increase of mononuclear cells, associated to lymphopenia, but erythrocyte sedimentation rate, seric cholesterol, transaminases, kidney function and total proteins were normal. Moderated thrombocytopenia and high levels of uric acid were observed. In the bone marrow aspiration atypical lymphocytes and relative depression of the hematopoietic series prevailed. Gastroscopy was negative. Skin biopsy: Compatible with Kaposi´s Sarcoma. HIV: three exams during the study at 3 months intervals being negative from the beginning. The last viral examination carried out at 7 months after the beginning of the study resulted positive when Western Blot technique was used. Key words: Kaposi´s sarcoma, HIV.
