Research in Family Medicine in Slovenia

SUMMARY

The primary level of health care represents an entry point into the health system, and the quality and economic sustainability of the healthcare system largely depend on it. Research is the driver of professional development of the best quality of patient care.Family medicine in Slovenia represents the largest specialty at the primary level of health care, which has achieved great success in the field of research in recent decades.The overview will show the characteristics of family medicine research, the inclusion of research into different levels of the educational process and practical work, as well as the indicated challenges that await family medicine as a scientific and research discipline in the future.

Se realizó una investigación aplicada, descriptiva, explicativa, longitudinal prospectiva, con el objetivo de caracterizar los principales factores de riesgo socio ambientales de las EDA en el área de salud, mediante la vinculación del estudiante de medicina con la familia del niño ingresado en el Hospital Pediátrico "Pepe Portilla" (año 2007-2008). El universo estuvo constituido por 731 niños, con un muestreo aleatorio simple de 252. Se utilizaron los métodos de la encuesta, análisis documental, observación mediante la visita a los hogares. Se procesaron los resultados mediante la estadística descriptiva y arribaron a los siguientes resultados: el estado nutricional detectado fue de 66 niños con afectación del estado nutricional y 186 sin afectación, vinculándose a los primeros las peores condiciones higiénicas sanitarias. En ambos grupos se detectaron deficiencias en las acciones de salud que deben ser instrumentadas en la atención primaria sobre prevención-promoción de las EDA y el seguimiento nutricional a lactantes; por lo que se realizaron charlas educativas por los estudiantes de Medicina sobre las orientaciones higiénicas epidemiológicas en la prevención de la diarrea. Se logró incorporar el componente investigativo en el proceso docente educativo de la formación del estudiante de Medicina.Palabras claves: EDA, acciones de salud, caracterización, factores de riesgo higiénico sanitarios, componente investigativoABSTRACTThe morbidity due to Acute Diarrheic Disease (ADD) still constitutes a health problem, though the figures of mortality have been reduced. An applied, descriptive, explanatory, longitudinal-prospective research was conducted with the purpose of characterizing the main socio-environmental risk factors of the ADD in the health area, linking the medical students with the family of the child admitted at "Pepe Portilla" Children Provincial Hospital (2007-2008). The target group was comprised of 731 children with a sample of 252 taken at random. The methods used were the survey, the documentary analysis and the observation through home visits. The results were processed using the descriptive statistics with the following results: 66 children with poor nutritional status in close relation to the worst hygienic conditions and 186 children without affectation. Deficiencies were detected in both groups when taking the health actions to be implemented in Primary Health Care regarding the prevention of ADD, health promotion work and a follow up of the nutritional status of the infants; that was why medical students organized educational talks about epidemiological-hygienic orientations to prevent the ADD. The investigative component about this topic was included in the teaching-learning process to train medical students.Key words: Acute Diarrheic Disease, health actions, characterization, sanitary hygienic risk factors, investigative component.

Estrategia preventiva para la adicción al alcohol en gemelos: Pinar del Río, 2007 / A strategy for the prevention of alcoholism in twins: Pinar del Rio, 2007Estrategia preventiva para la adicción al alcohol en gemelos: Pinar del Río, 2007 / A strategy for the prevention of alcoholism in twins: Pinar del Rio, 2007Con el objetivo de proponer una estrategia de prevención para la adicción al alcohol  en gemelos, se realizó una investigación epidemiológica, analítica y transversal (estudio en gemelos) en los meses de febrero y marzo del año 2007, en la provincia de Pinar del Río. El universo fue tomado del Registro Nacional de gemelos, estudiándose el comportamiento clínico, epidemiológico y genealógico de la adicción al  alcohol en gemelos. Para la recolección de la información se confeccionó una encuesta y un árbol genealógico para cada familia. Se calculó la concordancia en gemelos, para el par y para el caso índice y la heredabilidad. Los datos se presentaron en cuadros y gráficos. Se concluyó que la mayor cantidad de los pacientes se encontraron en el grupo de mayores de 40 años de edad,  del sexo masculino, en su mayoría dependientes alcohólicos. Los antecedentes familiares de alcoholismo resultaron estadísticamente significativos en el grupo de los familiares de primer grado. La concordancia fue mayor en gemelos monocigóticos que en  dicigóticos y la contribución genética para el alcoholismo de 66%. Se confeccionó una estrategia de prevención del alcoholismo basada en los principios del asesoramiento genético. Palabras clave: alcoholismo, gemelos/genética. ABSTRACT Aimed at proposing a strategy for the prevention of alcoholism in twins an epidemiological, analytical and cross-sectional research was carried out during February - March 2007 in Pinar del Rio province. The universe was taken from the National Records of twins, studying clinical, epidemiological and genealogical behavior of alcohol consumption in twins. To collect the information a survey and a genealogical tree for each family were made. Also calculating twin's concordance for the par, case rate and inheritance; data were presented in tables and graphics; prevailing alcoholic patients older than 40 years old and male sex. Familial history of alcoholism resulted statistically significant in the group of first degree relatives. The concordance was greater in monozygotic than in dizygotic twins and genetic contribution for the alcoholism reached 66%. A strategy based on the principles of genetic advice to prevent alcoholism was created. Key words: Alcoholism, Twins/geneticS.
