Maternal Relationship in Building the Identity of Twins

SUMMARY

Although little discussed, the theme “maternal relationship in twin situation” is extremely important, so that, although born and grow together, the twins have speci c needs that in uence the construction of their identities. In this way, this article sought through a literature review, explain and discuss the topic based on theoretical references published in books, magazines and periodicals. For both, devised to conduct a literature review for convenience in BVS scienti c database with the following descriptors (twins AND maternal care AND bond AND psychoanalysis). e results highlighted that, maternal care in twins situations has a certain complexity, once, this mother will have to care her two babies individually, intending to assist the construction of the identity of these. Besides, it is emphasized that during the process of individuation the babies need to deal with anxieties that permeate their lives, that is, they have the challenge to develop their identity separated not only from her mother, but also from his own brother.

Agregación familiar en niños con hipertensión arterial esencial/Family aggregation in children with essential hypertension Agregación familiar en niños con hipertensión arterial esencial/Family aggregation in children with essential hypertensionRESUMENObjetivo: identificar la influencia de los factores genéticos en la aparición de la hipertensión arterial esencial en niños.Materiales y Métodos: se realizó un estudio de casos y controles retrospectivo en pacientes pediátricos atendidos en el Hospital Naval de La Habana, desde enero de 2015 hasta septiembre de 2016; la muestra incluyó 45 casos y 90 controles para evaluar asociación entre diferentes variables cualitativas.Resultados: la hipertensión arterial esencial predominó en el grupo de 11 a 18 años (88,4%) y sexo masculino (75,6%), la obesidad y el sobrepeso estuvo presente en el 84,4% de los hipertensos, la agregación familiar se evidenció, cuando las madres presentaron hipertensión durante el embarazo pues sus hijos tuvieron una mayor probabilidad de padecer hipertensión (Odds ratio 4,16), si el padre era hipertenso el riesgo fue (OR 2,19), siendo también significativo; en la segunda generación se demostró la agregación familiar en los abuelos maternos (OR 2,19) y paternos (OR 3,23) de que sus nietos fueran hipertensos. En este estudio, la línea parental paterna tuvo una mayor relación con la agregación familiar y la probabilidad de aparición de hipertensión arterial esencial.Conclusiones: la agregación familiar y el grado de parentesco son factores genéticos que influyen en la aparición de estanenfermedad en las edades pediátricas, lo que demuestra la herencia de genes predisponentes.ABSTRACTObjective: to identify the influence of genetic factors on the appearance of essential hypertension in children.Materials and Methods: a retrospective case-control study was conducted in pediatric patients treated at the Naval Hospital of Havana, from January 2015 to September 2016; The sample included 45 cases and 90 controls to evaluate the association between different qualitative variables.Results: the essential arterial hypertension prevailed in the group of 11 to 18 years (88.4%) and male sex (75.6%), obesity and overweight were present in 84.4% of the hypertensive patients, the aggregation family was evident, when mothers had hypertension during pregnancy because their children were more likely to suffer hypertension (Odds ratio 4,16), if the father was hypertensive the risk was (OR 2.19), being also significant; in the second generation, family aggregation was demonstrated in maternal (OR 2.19) and paternal (OR 3.23) grandparents that their grandchildren were hypertensive. In this study, the paternal parental line had a greater relationship with family aggregation and the probability of the appearance of essential hypertension.Conclusions: family aggregation and degree of kinship are genetic factors that influence the appearance of this disease in pediatric ages, which demonstrates the inheritance of predisposing genes.

Alfafetoproteína elevada en gestantes de la provincia Artemisa / High alpha-fetoprotein in pregnant women from Artemisa province Alfafetoproteína elevada en gestantes de la provincia Artemisa / High alpha-fetoprotein in pregnant women from Artemisa provinceObjetivo: caracterizar la relación de la Alfafetoproteína elevada en suero materno con la aparición de defectos congénitos y complicaciones obstétricas en gestantes de la provincia Artemisa en el periodo de enero del año 2011 a diciembre del 2013.Método: se realizó un estudio descriptivo transversal en tres municipios, se consideraron elevadas las muestras con concentraciones de Alfafetoproteìna iguales o superiores a 2,0 MoM. Los datos fueron obtenidos del registro lineal de las consultas municipales de riesgo genético, así como los informes anuales del programa de Genética y procesados con técnicas de estadísticas descriptivas. Se tuvo en cuenta los principios éticos establecidos.Resultados: se encontró que solo el 3,5% de las gestantes con Alfafetoproteína elevada presentaron algún defecto congénito. En el 84% no se encontró una causa aparente y de estas el 42,26% tuvieron alguna complicaciónobstétrica.Conclusiones: predominaron las gestantes con Alfafetoproteína elevada sin causa genética aparente, más defectos del tubo neural que de la pared abdominal. Se presentaron complicaciones obstétricas en casi la mitad de las gestantes estudiadas, predominó el bajo peso al nacer y el parto pretérmino, lo que permite considerar el valor de esta glicoproteína como predictora de complicaciones obstétricas y de utilidad para la identificación de pacientes dealto riesgo.ABSTRACTObjective: to characterize the relationship of the elevated alpha-fetoprotein in maternal serum with the appearance of congenital defects and obstetric complications in pregnant women of the Artemisa province in the period from January 2011 to December 2013.Method: a cross-sectional descriptive study was carried out in three municipalities; samples with concentrations of Alfafetoprotein equal to or greater than 2.0 MoM were considered high. The data were obtained from the linear registry of the municipal genetic risk consultations, as well as the annual reports of the Genetics program and processed with descriptive statistics techniques. The established ethical principles were taken into accountResults: it was found that only 3,5% of pregnant women with high alpha-fetoprotein had a congenital defect. In 84%, no apparent cause was found and of these 42,26% had some obstetric complication.Conclusions: pregnant women predominated with high alpha-fetoprotein without apparent genetic cause, more defects of the neural tube than of the abdominal wall. Obstetric complications occurred in almost half ofthe pregnant women studied, low birth weight and preterm birth predominated, which allows us to consider the value of this glycoprotein as a predictor of obstetric complications and useful for the identification of high risk patients.Keywords: Alpha-fetoprotein; obstetric risk; obstetric complications.
