Clinical management of COVID-19: Experiences of the COVID-19 epidemic from Groote Schuur Hospital, Cape Town, South Africa

SUMMARY

The SARS-CoV-2 pandemic has presented clinicians with an enormous challenge in managing a respiratory virus that is not only capable of causing severe pneumonia and acute respiratory distress syndrome, but also multisystem disease. The extraordinary pace of clinical research, and particularly the surge in adaptive trials of new and repurposed treatments, have provided rapid answers to questions of whether such treatments work, and has resulted in corticosteroids taking centre stage in the management of hospitalised patients requiring oxygen support. Some treatment modalities, such as the role of anticoagulation to prevent and treat potential thromboembolic complications, remain controversial, as does the use of high-level oxygen support, outside of an intensive care unit setting. In this paper, we describe the clinical management of COVID-19 patients admitted to Groote Schuur Hospital, a major tertiary level hospital at the epicentre of South Africa’s SARS-CoV-2 epidemic during its first 4 months.

Pinar del RioSe realizó una investigación de desarrollo-aplicada, exploratoria, descriptiva y transversal, basada en diagnóstico previo por una investigación que permitió caracterizar la hipertensión portal (HTP) pediátrica y establecer importantes aspectos clínico-imagenológicos de la enfermedad en Pinar del Río. Además se efectuó el análisis documental de diversos sistemas de manejo de esta patología, adaptándolos al territorio, el objetivo fue elaborar una estrategia de manejo imagenológico de la HTP pediátrica a partir de la propuesta de un sistema de manejo diagnóstico integral, dirigido al personal de la salud de la Atención Primaria y Secundaria, establecer categorías de riesgo de padecer la HTP, permitiendo evaluar adecuadamente el estado de estos pacientes, desarrollando acciones de salud preventivo curativas, de valor como indicador de pronóstico, creando los mecanismos para lograr la profilaxis y el diagnóstico precoz y de esa forma disminuir las tasas de incidencia, morbilidad y mortalidad de la HTP, el índice de complicaciones graves como el sangrado digestivo o la encefalopatía portal y elevar la calidad de vida de estos niños.Palabras clave: hipertensión portal-estrategia-salud infantil, HIPERTENSIÓN PORTAL/diagnóstico, SALUD INFANTIL/tendencias, IMAGENOLOGÍA.ABSTRACTA development-applied, exploratory, descriptive and cross-sectional research was conducted, based on the previous diagnosis of a research characterizing Portal Hypertension in Pediatric Ages (PHPA). This research established important clinical-imaging aspects of the disease in Pinar Rio. Besides; a documentary analysis about diverse systems in the management of this pathology was carried out adapting them for the territory. The purpose was to create a strategy for the imaging management of the PHPA starting from a proposal of a system to an integral diagnostic management, and targeted at Primary and Secondary Health Care personnel in order to establish the risk categories of suffering from this disease. It also allows, a proper assessment of these patients' status, the development of preventive, curative health actions, knowing the value as an indicator of prognosis, creating a mechanism to achieve the prophylaxis and the early diagnosis as ways to diminish PHPA incidence, morbidity and mortality rates as well as the rate of severe complications such as digestive bleeding or portal encephalopathy aimed at increasing the quality of life of these children.Key words: Portal hypertension/strategy/diagnosis, Children health/trends, Imaging.

Anomalía vaginal: a propósito de un caso interesante / Vaginal abnormality: an interesting case reportAnomalía vaginal: a propósito de un caso interesante / Vaginal abnormality: an interesting case reportLas malformaciones uterovaginales (MUV), se deben a una alteración en el proceso de  embriogénesis del sistema reproductor femenino. La ecografía tiene un rol importante en el estudio inicial de las mismas, la RMN tiene ventajas importantes, es un estudio no invasivo, con una sensibilidad de casi 100%. La histerosalpingografía es la técnica clásica para el estudio de las MUV que proporciona información sobre el canal cervical, endometrial  y la permeabilidad tubárica. Se demuestra la utilidad de la ecografía transabdominal en el manejo imagenológico de una MUV  y sus complicaciones principales al presentar el caso de una paciente de 12 años de edad que acudió a consulta con fiebre elevada,  dolor hipogástrico y oliguria/disuria sin  menarquia. Al examen físico se palpó  tumoración suprapúbica, abombamiento perineal e himen no tunelizado, se constató edema facial e hipertensión arterial. La ecografía reveló una moderada dilatación de los sistemas excretores de ambos riñones, la cavidad vesical notablemente distendida, visualizándose masa hipoecoica retrovesical en fosa ilíaca derecha,  sin septos ni contenido,  relacionada con hematometra secundaria a himen imperforado, causando hidronefrosis obstructiva bilateral que condujo a  retención urinaria y de fallo renal agudo. Se realizó corrección quirúrgica de la MUV obteniéndo una evolución clínico imagenológica satisfactoria.Palabras clave: ANOMALÍAS GENITOURINARIAS, HEMATOCOLPOS, ULTRASONOGRAFÍA, HEMATÓMETRA.ABSTRACTUterovaginal malformations (UVM) are due to an alteration in the process of embryogenesis of the female reproductive system. The imaging has a very important role during initial studies of these abnormalities; magnetic resonance imaging has significant advantages in non-invasive studies with a sensitivity of almost 100%. Hysterosalpingography is the classic technique to the study of UVM that provides information about cervical, endometrial canals and tubal permeability. The usefulness of the transabdominal imaging in the management of a UVM and main complications was showed when presenting a case of a 12 years old patient attending to the office with high fever, hypogastric pain and oliguria/dysuria without menarche. At physical examination a suprapubic tumor was palpated, a perineal dome, a non-tunneled hymen, facial edema and hypertension were observed. Imaging revealed a moderate dilatation of the excretory systems in both kidneys, vesical cavity was notable distended, visualizing a retrovesical hypoecoid mass in the right iliac fossa, without septa or contents related to a hematometra due to an imperforate hymen provoking bilateral obstructive hydronephrosis which led to a urinary retention of acute renal failure. Surgical correction of the UVM was carried out having a satisfactory imaging-clinical progression. Key words: UROGENITAL ABNORMALITIES, HEMATOCOLPOS, ULTRASONOGRAPHY, HEMATOMATRA.

Nefrolitiasis de infección: caso clínico / Nephrolithiasis of infection: clinical caseNefrolitiasis de infección: caso clínico / Nephrolithiasis of infection: clinical caseSe presenta un caso de litiasis de infección, conocida con el nombre de cálculos de estruvita y asociado a una enfermedad metabólica subyacente y diabetes mellitus tipo 2 de reciente debut, el cual se manifestó con oliguria e infección, como resultado de una obstrucción urinaria bilateral parcial. El paciente de 48 años de edad, con antecedentes de cardiopatía isquémica, hipertensión arterial y de expulsar cálculos de riñón, consultó su caso el 21 de enero de 2009. Manejaba cifras de creatinina, glicemia y acido úrico normales anterior al ingreso y estas tuvieron un comportamiento ascendente durante la obstrucción, excepto el ácido úrico. En el diagnóstico aportaron datos de interés la ecografía renal y de próstata, la Uro-TAC y gammagrafía renal con MAG-3. El diagnóstico diferencial incluyó toda causa extrínseca e intrínseca, esta última relacionada con cálculos, coágulos o papila renal necrosada, que provocaran alteración al libre flujo urinario. Se estableció como patrones de buena evolución, la recuperación del volumen urinario, la normalización de la glucemia y creatinina, así como la permanencia del urocultivo  negativo. El manejo terapéutico motivó a la utilización de la nefrolitotomía percútanea como una de las opciones que ofrecen los procederes endorulógicos, además del control de las enfermedadesde base,lo que resultó una favorable evolución del caso.Palabras clave: LITIASIS RENAL, DIAGNÓSTICO, OBSTRUCCIÓN URETRAL / tratamiento.ABSTRACTA clinical case of lithiasis of infection is presented, known as calculi of struvite associated to an underlying metabolic disease and diabetes mellitus type-2 of recent onset, with oliguria and infection as a result of a partial bilateral urinary infection. A 48 years-old patient having medical records of ischemic heart disease, hypertension and expulsion of kidney stones attended to the office in January 21, 2009 presenting normal values of creatinine, glycemia and uric acid before the admission; these values increased during the obstruction, except the uric acid. Renal and prostate imaging, the Uro-Computerized axial tomography and renal scintigraphy with MAG-3 showed data of interest. Differential diagnosis included extrinsic and intrinsic causes, the last one related to calculi, clots or renal necrotic papilla causing disorder of the free urinary flow. Patterns of good progress were established: recovery of the urinary volume, normal levels of glycemia and creatinine and a permanent negative uroculture. Therapeutic management motivated the use of percutaneous nephrolithotomy as one of the choices for endourologic procedures, as well as the control of the underlying diseases, being favorable for a good progress of the case.Key words: NEPHROLITIASIS, DIAGNOSIS, URETHRAL OBSTRUCTION/treatment.
