Forensic medical analysis of the domestic violence against women attempts to the emergency department: Izmir caseAcil servise basvuran kadina yönelik siddet olgularina adli tibbi bakis: Izmir örnegi

SUMMARY

Objective: With our study, followed by interviews with victims of domestic violence against women cases directed to our forensic department; sociodemographic characteristics  of the victims, the nature of their exposure to violence, investigate the trauma they experienced and to have tried to behave more attentive to their health care these patients are evaluated, expression the need to pass a special training process for this area.Method: This research is a descriptive study that has been carried out on 156 women applying to the Emergency Department of Izmir Katip Çelebi University Atatürk Training and Research Hospital after being exposed to the domestic physical violence, and directed to the Forensic Medicine Polyclinic in 2013 so that the legal authorities could draw up their forensic reports subsequent to the notification of forensic cases.The questions in the “Registration Form for Domestic Violence against Women” drawn up by the Prime Ministry of the Republic of Turkey were used during the interviews.The steps such as interviewing with the subjects, recording the data on the forms, briefing the victims about violence and developing a security plan were taken together with an experienced forensic nurse and a forensic medicine specialist who have received education in this field.Current data recorder in the computer and analyzed with SPSS for Windows 15.0 statistical software. In the statistics, the data obtained from the records in the form of the variables; the number, frequency, average, minimum and maximum values are calculated.Findings: To give an example of the remarkable results found in our study; when women are admitted to the emergency department 5.1% is pregnant, 28.2% were subjected to sexual violence, 83.3% had been threatened with death, 17.3% of the firearms kept at home, 12.4% attempt as a result of the burns, serious injuries such as fractures, 92.3% of the increase in recent years of violence, 55.1% the violent has substance abuse, 90.6% of children are also exposed to violence and 93% stated that children witnessed the violence.Result: The findings obtained in this study demonstrate the significance of the education for medical professionals working in this field so that the women exposed to the domestic violence can benefit from qualified diagnosis, treatment and rehabilitation processes. It is an indisputable reality that this issue makes great contribution to the social rehabilitation processes as well as the provision of qualified diagnosis and treatment for the victims. ÖzetAmaç: Çalismamiz, adli tip poliklinigimize yönlendirilen kadina yönelik siddet magduru olgular ile yapilan görüsmelerin ardindan; kisilerin sosyodemografik özellikleri, maruz kaldiklari siddetin nitelikleri, yasadiklari travmanin etkileri irdelenmeye ve bu olgulari degerlendiren saglik çalisanlarinin daha özenli davranmasinin, gerekirse bu alana yönelik özellikli bir egitim sürecinden geçmesinin gerekliliginin belirtilmesi amaciyla planlanmis olup, tanimlayici nitelikte bir çalismadir.  Yöntem: Çalismamiz, aile içinde siddet görmeleri sonrasinda Izmir Katip Çelebi Üniversitesi Atatürk Egitim ve Arastirma Hastanesi Acil Servisi’ne basvuran ve adli olgu bildirimlerinin yapilmasinin ardindan, adli raporlarinin düzenlenmesi için 2013 yili içerisinde Adli Tip Poliklinigi’mize yönlendirilen 156 kadin ile gerçeklestirilen, tanimlayici nitelikte bir çalismadir.Olgular ile görüsmeler adli hemsire ve adli tip uzmani ile birlikte gerçeklestirilmistir. Görüsmeler sirasinda; Basbakanlik tarafindan hazirlanan “Kadina Yönelik Aile Içi Siddet Kayit Formu”ndaki sorulardan yararlanilmistir. Mevcut veriler, bilgisayar ortamina girilmis ve SPSS for Windows 15.0 istatistik programi ile analiz edilmistir. Yapilan istatistiklerde kayit formunda yer alan degiskenlerden elde edilen verilerin sayisi, frekansi, ortalama, en küçük ve en büyük degerleri hesaplanmistir.Bulgular: Çalismamizda saptanan dikkat çekici sonuçlara örnek vermek gerekirse, acil servise basvurularinda kadinlarin; %5,1’i gebe oldugunu, %28,2’si cinsel siddete maruz kaldigini, %83,3’ü ölümle tehdit  edildigini, %17,3’ü evlerinde atesli silah bulundugunu, %12,4’ü siddet sonucunda yanik, kemik kirigi gibi ciddi yaralanmalar ile basvurdugunu, %92,3’ü siddetin son bir senede artis gösterdigini, %55,1’i siddet uygulayanin madde bagimliligi oldugunu, %90,6’si çocuklarin da siddet gördügünü ve %93’ü çocuklarin yasanan siddete taniklik ettigini  ifade etmistir. Sonuç: Elde edilen bulgular; aile içi siddet magduru kadinlarin nitelikli tani, tedavi ile rehabilitasyon süreçlerinin yerine getirilebilmesi için bu alanda çalisan saglik personelinin egitimli olmasinin önemini göstermektedir. Ayrica nitelikli muayeneyle elde edilecek bulgular ile düzenlenen raporlarin, iletildigi adli makamlar tarafindan degerlendirilmesiyle hak kayiplarinin önüne geçilmesi saglanacaktir.
