Hypoxic ischemic encephalopathy with sensory neural hearing loss as a complication: A 24-month follow up after the initiation of hypothermia therapy

SUMMARY

Perinatal asphyxia is one of the most common causes of newborn mortality, with an incidence of two to five cases per 1,000 live births in developed countries and tenfold in developing countries.1,2 Lack of oxygen at birth may cause hypoxic ischemic encephalopathy (HIE) with severe neurological consequences, such as cerebral palsy, global developmental delay, blindness or visual defects, hearing loss or deafness, and other comorbidities. Hypothermia therapy is currently the only management option for HIE included in neonatal intensive care unit (NICU) protocols.3We report here a 24-month follow up of a full-term infant with moderate HIE who underwent hypothermia therapy for 72 hours. The patient had moderate sensorineural hearing loss (SNHL) at her first brainstem evoked response audiometry (BERA) examination at 5 months of age, but had normal hearing and neurodevelopment after 24 months of follow-up.

Hypoxic Ischemic Encephalopathy in Term new BorsnsSe realizó un trabajo transversal y prospectivo en el Hospital Docente Ginecoobstétrico de Pinar del Río "Justo Legón Padilla" desde enero de 1995 a febrero 1996 que intereso a todos los recién nacidos a término que desarrollaron encefalopatía hipóxica isquémica. Se estudiaron las variables edad, tiempo de gestación, paridad y peso del recién nacido. Se utilizaron las pruebas de chi cuadrado y el test de probabilidad exacta de Ficher como prueba de confiabilidad estadística (alfa = 0.05) resultando dependientes las variables edad (p< 0.05) y el tiempo de gestación (p< 0.05) y el tiempo de gestación (p< 0.001) al comparar el grupo A, de igual forma el peso tuvo también significación estadística (p< 0.05) a favor de los recien nacidos menor que 2.500 gramos. DeCS: RECIEN NACIDO, ANOXIA, ASFIXIA NEONATAL, ISQUEMIA CEREBRAL.A prospective cross-sectional study about term newborns with ischemic encephalopathy was carried out at Justo Legon Padilla Gyneco-Obstetric Hospital, between January 1995 and February 1996. Age, gestational period, parity and weight of the newborn were the variables of the study. Chi-squared test and Fisher's exact probability test were used for statistical reliability (alfa=0.05) resulting in a dependent corelation between age (p<0.05) and gestational time (p<0.001) when comparing group B; as well as nulliparity (p<0.05) when comparing group A. Weight was also statistically significant (p<0.05) for newborns >2.500 g. DeCS: ANOXIA, ASPHYXIA NEONATORUM, CEREBRAL ISCHEMIA.

A sporadic case presentationLa obstrucción intestinal duodenal es frecuente en el recién nacido como causa de oclusión, puede ser completa (atresia duodenal) secundaria a un trastorno de revacuolización de la luz intestinal o por  una rotación irregular del páncreas hacia la derecha del duodeno. Presentamos a un recién nacido  con edad gestacional de 35.4 semanas, conteo de Apgar de 8-8 puntos, que nace con signos de shock hipovolémico secundario a una anemia aguda por desgarro del corión, con una Encefalopatía Hipóxica Isquémica(EHI). Desarrolló posteriormente una intolerancia digestiva con abundante residuo gástrico, valorándose una oclusión intestinal incompleta alta, posiblemente por una estenosis duodenal. Se le realizan radiografías de abdomen simple de pie apoyándose la sospecha clínica por la presencia de imagen típica "en doble burbuja", se realizó laparotomía exploradora encontrando en el acto quirúrgico  una Estenosis Duodenal secundaria a un Páncreas Anular. Es egresado a los 45 días de vida con buen peso corporal y reflujo gastroesofágico grado III como complicación. Actualmente se encuentra  bajo seguimiento ambulatorio multidisciplinario y en franca evolución satisfactoria. Palabras clave: Estenosis Duodenal, Obstrucción Intestinal, Páncreas Anular. ABSTRACT Duodenal intestinal obstruction is frequent in newborn as a cause of occlusion, it can be complete (duodenal atresia), secondary to a disorder of revacuolization of the intestinal lumen, or due to an irregular rotation of pancreas towards theright of duodenum. A newborn with a gestational age of 35.4 weeks, apgar score of 8.8 who was born with signs of hypovolemic shock secondary to an acute anemia caused by chorion tear with an Ischemic Hypoxic Encephalopathy, who subsequently deveoped a digestive intolerance with abundant gastric residue. An incomplete intestinal occluson was assessed - possibly as a result of a duodenal stenosis - simple abdominal X - rays was performed, supporting clinical suspicion because of the presence of a typical image in "double bubble". An exploratory laparatomy was carried out, finding in surgery a Duodenal Stenosis secondary to an annular Pancreas. The patient was discharged from the hospital 45 days after being born with a good weight and as complication a gastroesophageal reflux of third degreee. Currently the patient undergoes ambulatory-multidisciplinary follow-up, and is making a satisfactory progress. Subject headings: DUODENAL STENOSIS, INTESTINAL OBSTRUCTION, ANNULAR PANCREAS.
