Evaluation of postgraduate family medicine trainee’s knowledge and attitude following an online Educational Module in Palliative Care: A descriptive studyOnline Educational Module in Palliative Care

SUMMARY

Objectives: To identify improvement in knowledge and attitude of Family Medicine (FM) postgraduate trainees (PGT) towards Palliative care (PC) in order to provide effective care to the patients with advanced disease.Methods: A cross-sectional study was conducted over eight weeks from 1st July till 3rd September 2021 at Family Medicine Department, Aga Khan University Hospital (AKUH). PGT who willingly signed the written informed consent were enrolled in the study. Descriptive analysis, frequencies, proportions and thematic approach were used for data analysis. Data was analyzed using SPSS version 23.Results: FM-PGT were included in the study. Improvement in knowledge was observed in posttest scores along with positive change in their attitude and improved perception of level of confidence for managing PC patients. Overall assessment of PCM was positive.Conclusion: This PCM seems to be a useful tool for PC training in postgraduate medical education (PGME). Propuesta metodológica de una clase en educación en el trabajo / A methodological proposal for a lecture about "Education in Work"Propuesta metodológica de una clase en educación en el trabajo / A methodological proposal for a lecture about "Education in Work"La contradicción fundamental, en el objeto de la Didáctica se presenta entre las exigencias que la sociedad plantea en la enseñanza y el nivel alcanzado por los estudiantes en un momento dado durante su aprendizaje. La solución de esta contradicción es el motor impulsor de este proceso. En aras de contribuir con él, nuestro objetivo fue mostrar una propuesta metodológica de una clase en educación en el trabajo en la formación del Especialista en Estomatología General Integral. En el contenido del programa analítico, están bien establecidos los objetivos por año académico y por módulos, no así por temas. Para elaborar el objetivo temático se tuvo en cuenta su función: lograr transformaciones graduales en los sistemas de conocimientos y habilidades de los residentes, así como en sus actitudes, convicciones, sentimientos, ideales y valores. Se prestó gran atención a los métodos problémicos, así como al método clínico en su dimensión didáctica. Este enfoque favorece el paradigma educacional actual de aprender a aprender y de aprender haciendo bajo la conducción del que enseña. Se elaboró un sistema de tareas docentes. Esta tarea constituye un modo de concretar el objeto de estudio. Estimula independencia cognoscitiva, individualidad creadora, integridad, carácter consciente y activo. Se concluye que el asumir esta premisa aumentó la responsabilidad del profesor en cuanto a pensar en términos de objetivo, de manera tal que permita al residente ir más allá de un mero aprender de asociaciones, a un generar de ideas propias. ABSTRACT The main contradiction in the object of Didactics is between the demands the society establishes in learning and the level students achieve at a given moment along the teaching-learning process. The solution for this contradiction is the motivation of this process. For the purpose of contributing to this process, a methodological proposal for a lecture about " Education in Work " was created to form Comprehensive Specialists in Dentistry. In the content of this analytic syllabus the objectives are properly set up per academic years and modules, but not per themes. The thematic objective was created taking into account its function: to achieve gradual transformations in the systems of knowledge and skills in residents; as well as in the attitudes, convictions, feelings, ideas and values. Problematic methods were carefully observed and a clinical method was analyzed from a didactic point of view. This approach favours current educational paradigm - learning to learn and learning doing - under the guide of whoever teaches. A system of teaching tasks was prepared. These tasks constitute a way to pinpoint the object of study. It motivates cognitive independence, a creative individual, integrity, active and conscious character. Concluding that, when this approach was adopted, professor's responsibility towards thinking of objective terms increased, allowing this way , that the residents in the specialty go beyond learning through associations to create own ideas. Valoración integral del examen estatal de la residencia en estomatología general integral curso 2005-2006 / Comprehensive Assessment on the State exam of Comprehensive Dental Care Residency. 2005-2006Valoración integral del examen estatal de la residencia en estomatología general integral curso 2005-2006 / Comprehensive Assessment on the State exam of Comprehensive Dental Care Residency. 2005-2006El examen estatal es una evaluación que comprueba el logro de los objetivos generales y esenciales del contenido, teniendo en cuenta que se realiza al finalizar el segundo año de la especialidad de Estomatología General Integral, después de haber aprobado todos los módulos comprendidos en el programa de la residencia, al concluir el curso académico (2005-2006). Se realizó una investigación descriptiva y retrospectiva a 134 exámenes estatales aplicados a los residentes de estomatología general integral de la Facultad de Ciencias Médicas de Pinar del Río, cuyo objetivo principal fue hacer una valoración integral del instrumento evaluativo. Se revisó la totalidad de los exámenes y a los datos obtenidos se les aplicó el análisis porcentual de frecuencia relativa encontrando que los estudiantes obtuvieron mejores calificaciones en el examen práctico en el intervalo de 80 _ 89 y de 90- 95, mientras que en el examen estatal escrito la mayoría de los residentes obtuvieron calificaciones en el rango de 70 -79. Al analizar las mayores dificultades en las preguntas del examen determinamos que el mayor número de errores estuvo en las preguntas de crecimiento, desarrollo y atención al menor de 19 años II , no existiendo preguntas del módulo de promoción, educación para la salud y prevención, ni en el de atención al paciente discapacitado, a pesar de ser contenidos muy importantes para dar salida a los objetivos terminales, y por ende, al perfil del egresado del verdadero especialista de Estomatología General Integral que demanda nuestro Sistema Nacional de Salud. ABSTRACT The state exam is an evaluation which documents the accomplishing of the general and essential content objectives, considering that it is applied at the end of the second year of the specialty of Comprehensive Dental Care, after having passed all the modules comprised in the residency syllabus when the academic year (2005-2006) ended. A descriptive and retrospective research was performed with 134 state exams applied to the residents in Comprehensive Dental Care belonging to the Medical School in Pinar del Río, whose main objective was to carry out a comprehensive assessment of the evaluative tool. All the exams were reviewed and the data collected underwent the percentage analysis of relative frequency, finding that the students had the best qualifications in the practical exam in the interval of 80-89 and 90-95, whereas, in the written state exams, most residents had qualifications ranging 70-79. When analyzing the greatest difficulties in the questions of the exam we found that the greatest number of mistakes lied in questions on growth and progress of and assistance for 19-year-old minors, with no questions of promotion, health education, and prevention modules, nor in the assistance for the disabled, in spite of that content being very important to accomplish the final objectives, and therefore the graduate's, the real comprehensive dental care specialist's profile that our National Health Systems demands.
