PROGNOSTICATION OF THE FUNCTIONING EFFICACY OF INFRAINGUINAL SHUNT IN PATIENTS, SUFFERING CRITICAL ISCHEMIA OF THE LOWER EXTREMITY TISSUES

SUMMARY

Objective. To determine prognostic criteria of functioning of artificial distal femoro–popliteal and femoro–tibial shunts in accordance to the triplex scanning (TS) data; to prove the necessity of the TS conduction in immediate postoperative period after reconstructive procedures and its impact on result of the lower extremity revascularization.??terials and methods. Results of operative treatment of the patients two groups are analyzed. In 481 patients (control group) shunting operations without TS–control of the shunt work were performed in early postoperative days; in 58 (the main group) after reconstructive operation a ?S–control was conducted necessarily.Results. In all the patients operation of infrainguinal autovenous shunting for chronic critical ischemia of the lower extremities tissues of atherosclerotic genesis was performed. Three kinds of blood flow along the shunt, according to the TS data, were estimated in the main group with prognostic objective, depending on the their spectrum type, the wave amplitude, and the resistance index value.Conclusion. The ?? application in immediate postoperative period, formation of additional shunts or patches have permitted to reduce the shunt thrombosis occurrence by 5.3% (from 20.8 to 15.5%), rate of the operation poor result – by 7.8% (from 25 to 17.2%), and to enhance a positive result rate achievement – by 7.8% (from 75 to 82.8%).
