Amniocentesis increases level of anxiety in women with invasive prenatal diagnosis of Down syndrome

SUMMARY

Backgound Invasive prenatal diagnosis (PND) through amniocentesis and chorionic villus sampling (CVS) can detect Down syndrome. Pregnant women usually experience a variety of psychological responses associated with invasive PND. This study is intended to assess depression, anxiety and stress levels and the factors related to their psychological responses in pregnant women with invasive prenatal diagnosis of Down syndrome.Methods A cross sectional study was conducted at Kandang Kerbau Women’s and Children’s Hospital, Singapore. The psychological responses of 70 women undergoing PND were assessed by Depression Anxiety Stress Scale 21 (DASS 21) questionnaire. A multiple linear regression analysis was used to analyze association between knowledge and perceived risk with psychological responses (CI 95% and significance value p<0.05).Results More than half of the participants had normal anxiety (55.7%), stress (72.8%), depression levels (65.8%). The results revealed significantly higher level of anxiety in women with gestational age >13 weeks who had pursued amniocentesis. Women with no previous children had higher levels of depression and stress. Women who pursued amniocentesis had significantly higher anxiety scores compared to women undergoing CVS (p=0.015).Conclusions Women’s psychological responses are associated with gestational age, type of procedure and parity. The level of anxiety increased in women who underwent amniocentesis for diagnosis of Down syndrome. Knowledge and perceived risk of having a baby with Down syndrome do not seem to have psychological effects to women.

Reina Clinic, 1997Se realizó una investigación analítica transversal, con el objetivo de determinar el estado funcional de la población de 60 años y mas del Consejo Popular "Los Sitios". Se tomó una muestra aleatoria de cinco consultorios, utilizándose para el diseño de la misma un nivel de significación del 95 % La muestra calculada fue de 412 ancianos. El instrumento empleado, fue "El Examen Periódico de Salud", que incluye la Escala Geriátrica de Evaluación Funcional (EGEF). Se analizaron las variables: edad, sexo, estado marital, ocupación, nivel cultural, que fueron contrastadas, con el resultado obtenido de los items que evalúan el nivel de funcionabilidad, en las esferas biomédicas, psicológica y social. Se concluyó que la mayor prevalencia de deficiencias correspondió al uso de drogas, la memoria y el sueño. Todas las deficiencias aumentaron con la edad, unas de manera significativa, y otras no. Todas las mujeres presentaron mayores deficiencias que los hombres, excepto en lo referente a la audición. Estas diferencias, solo fueron significativas para los trastornos del sueño y estado emocional alterado. La mayoría de las deficiencias se encontraron en mayor proporción en las personas sin pareja. La población que no trabaja presento mayor prevalencia para todos los items, excepto para la audición. El nivel educacional se asocio a la tenencia de deficiencias en la esfera psicológica, menos en la memoria. DeCS: ANCIANO/GERIATRIA/ENVEJECIMIENTO. A transversal analytic investigation aiming to determine the funtional condition in a 60 year and older population was carried out in the popular council "Los Sitios". An aleatory sample of home offices was taken: a 95% significance level was used its design. Four hundred and twelve elder persons conformed the sample. The periodical Health Monitoring was the approach used, including the Functional Evaluation Geriatric Scale (FEGS). Age, sex, marital status, ocupation and cultural background were the variables used in contrast to previous item results evaluating funtional level on biomedical,psychological and social sphere. It was concluded that drugs, memory andsleep had the highest deficiency prevalence. Aging increased the deficiencies, in some cases significantly. These deficiencies were more common in women than men, excluding problems with audition. The differences were only significant for sleeping disorders and emotional alterations. Most of the deficiencies were frequently found in single persons. Unemployed population was associated to a higher prevalence for all the items, excluding problems with audition. The educational background was associated to the psychological sphere ,deficiencies, excluding memory. DeCS: AGED, GERIATRICS, AGING.
