Retained Surgical Items in Inguinal Canal: A Case Report and Literature Review

SUMMARY

BACKGROUND: Retained surgical items (RSI) are rare medical challenges with serious complications and medicolegal implications. Knowledge and preventive measures for these rare events are currently not sufficient to limit their increasing incidence. Gauzes and sponges constitute most of RSI. Forceps, needles and pins may be found too. Diagnosis of these events is challenging and often missed due to nonspecific clinical findings.PRESENTATION OF CASE: We present here a 49-year-old patient who presented to the clinic with a history of chronic scrotal sinus on the same side of a repeatedly repaired inguinal hernia 4 months before admission. He underwent exploration of the inguinal canal as elective surgery. Exploration of the inguinal canal revealed missed surgical gauze left during the previous hernia repair. The gauze was removed, and the inguinal canal was repaired. The postoperative period was uncomplicated.CONCLUSION: Retained surgical items are completely preventable near-events. Although they are rare entities, clinicians must have a high index of suspicion for any postoperative, in patients presenting with pain, sinus or palpable masses.

Closure of surgical wound per second intention in third inferior retained molarSe realizó un estudio en 40 pacientes con terceros molares inferiores retenidos que fueron atendidos en el servicio de Cirugía Maxilofacial del Hospital Docente Clínico Quirúrgico "Abel Santamaría" en el periodo de tiempo comprendido entre el 10 de septiembre de 199 y el 10 de febrero del año 2000 con el objetivo de valorar la evolución postoperatoria encontrada en los pacientes operados de terceros molares inferiores retenidos con cierre de la herida quirúrgica por segunda intención, atendiendo a la presencia de dolor, edema, sangramiento, ni sepsis ni trismo mandibular. Se plantean y analizan los resultados obtenidos estableciendo comparaciones con estudios anteriores. En los pacientes operados encontramos una favorable evolución postoperatoria, existiendo un 85 % de los casos sin dolor y un 90 % con edema ligero a las 72 horas de operados; no apareciendo casos con sangramiento, ni sepsis ni trismo mandibular. Esta técnica de tratamiento sin sutura de la herida quirúrgica permite a nuestros pacientes una rápida recuperación e incorporación social. DeCS: DIENTE IMPACTADO/ Cirugía, TERCER MOLAR/ Cirugía. ABSTRACT A study was carried out in 40 patients with retained inferior third molars who were cared at the Maxilofacial Surgery Service at Abel Santamaria Clinical Surgical Teaching Hospital in the period from September 10th. 1999 and February 2000 aimed at the post - operative evaluation found in patients operated upon of retained inferior third molar with closure of the surgical wound by second intention, according to the presence of pain, edema, bleeding, sepsis and mandible trismus. Results obtained are analyzed establishing comparisons with previous studies. A favorable postoperatory evolution was found in the operated patients, 85 5 of the cases did not have any pain and 90 % with light edema 72 hours after the surgery, no bleeding was reported, neither mandible trismus. Se plantean y analizan los resultados obtenidos, estableciendo comparaciones con estudios anteriores. En los pacientes operados mediante cada una de las técnicas descritas, encontramos una mejor evolución postoperatoria mediante la técnica con cierre de la herida quirúrgica por segunda intención, existiendo un 45% de los casos sin dolor y un 90% con edema ligero a las 72 horas de operados, no apareciendo casos con sangramiento, ni sepsis, ni trismo mandibular, presentando esta técnica de tratamiento una mayor efectividad en comparación con la técnica quirúrgica convencional. ABSTRACT A sample of 120 patients with lower semi-impacted third molars in the maxillofacial Surgery Department of Abel Santamaría General Hospital between September 10/1999-June/2000 was studied with the purpose of assessing the effectiveness of the surgical technique using the secondary healing suture, compared to the conventional surgical technique in patients with lower semi-impacted third molars according to the postoperative evolution (pain, edema, mandibular trismus, sepsis and bleeding). Results are analyzed establishing comparisons with prior studies. The surgical patients upon who any of the previous procedures had been used were found to have a better evolution with a secondary healing suture. Pain was absent in 45% cases and a mild edema was observed 72 hours postoperatively. Bleeding, sepsis and mandibular trismus were not observed, so this technique represents a better effectiveness versus the conventional surgical one.
