The effect of vaginal delivery and Caesarean section on the anal Sphincter complex of Primipara based on optimized three-dimensional ultrasound image and nuclear regression Reconstruction Algorithm

SUMMARY

Objective: The study used the optimized nuclear regression reconstruction algorithm to explore the value of three-dimensional perineal ultrasound evaluation of the effect of caesarean delivery and caesarean section on the anal sphincter complex of primipara.Methods: This study performed three-dimensional perineal ultrasound scanning of the anal sphincter complex of 157 primiparas 42 days after delivery. Among them, 77 were in caesarean delivery (spontaneous delivery group) and 80 were in caesarean section (caesarean delivery group) from September 2018 to December 2020 in our hospital. The thickness of the end plane, the middle plane, the distal plane and the distal plane of the external anal sphincter at 3, 6, 9, 12 o’clock direction, and measure the thickness of the central plane of the pubic rectum muscle at 4, 8 o’clock direction. At the same time, the study used tomography and volume contrast imaging to observe the morphology and integrity of the anal sphincter complex.Results: The thickness of the distal anal sphincter at the 12 o’clock direction, the proximal anal sphincter at 6, 12 o’clock, and the central plane at 9 and 12 o’clock in the obstetric group were smaller than those in the caesarean section group (all P < 0.05). There were no significant differences in the thickness of the remaining anal internal and external anal sphincter and puborectalis muscles between the two groups in different directions (all P>0.05). In the obstetric group, a perineal sphincter defect was found via three-dimensional perineal ultrasound.Conclusion: The delivery method has a certain influence on the shape of the anal sphincter complex. The thickness of the internal and external anal sphincter of the primiparous women in a certain direction is significantly smaller than that of caesarean section. Transperineally three-dimensional ultrasound can clearly show the morphological characteristics and integrity of the anal sphincter complex, and diagnose the defect of the anal sphincter complex.doi: https://doi.org/10.12669/pjms.37.6-WIT.4859How to cite this:He N, Shi L. Efectividad del manejo sindrómico para reducir las complicaciones de las infecciones de transmisión sexual / Effectiveness of the syndromic management for the reduction of complications in sexually transmitted InfectionsEfectividad del manejo sindrómico para reducir las complicaciones de las infecciones de transmisión sexual / Effectiveness of the syndromic management for the reduction of complications in sexually transmitted InfectionsSe realizó un estudio de cohorte, con el objetivo de evaluar la efectividad del manejo sindrómico en la reducción de las complicaciones producidas por las infecciones de transmisión sexual en el municipio San Cristóbal, durante los meses de enero de 2005 a diciembre de 2006. La muestra estuvo constituida por el total de pacientes que solicitaron atención médica por presentar secreción uretral, flujo vaginal y/o dolor abdominal bajo en este período. Los datos fueron recogidos a través de encuestas, hojas de cargo y registros hospitalarios. Se efectuó el cálculo de por ciento, tasa y pruebas de significación estadística según procedió. Se concluyó que al inicio del estudio el mayor por ciento de los médicos poseía bajos conocimientos sobre el tema. La enfermedad inflamatoria pélvica aguda fue la complicación más frecuente encontrada antes y después de aplicado al estudio, siendo notoria la reducción que experimentaron todas estas patologías al concluir el mismo, resultando significativo los resultados encontrados. La mayoría de los pacientes de la cohorte A (95,8%) resolvieron su sintomatología antes de los siete días después del tratamiento. ABSTRACT A cohort study aimed at assessing the effectiveness of the Syndromic Management for the reduction of complications produced by Sexually Transmitted Infections was conducted in San Cristobal municipality during both, January 2005 and December 2006. The sample was comprised of the total of patients requesting medical assistance for presenting urethral discharge, vaginal discharge and/or low-abdominal pain during this period of time. Data were collected by means of surveys, charge-sheet and hospital registers. Percentage calculation, rate and statistical significance testing were applied when proceeding. Concluding that, at the beginning of the research a greater percent of doctors had a low level of knowledge about this topic. Acute Pelvic Inflammatory Disease was the most frequent complication found before and after the study; being noticeable the reduction of all these pathologies when the study was concluded. The majority of the patients included in the cohort-A (95, 8%) solved the symptoms before seven days after the treatment.
