Pengukuran Tingkat Kepatuhan Pengobatan Pasien Hipertensi di Fasilitas Kesehatan Tingkat Pertama di Kota Bandung

Abstract


Hipertensi merupakan salah satu penyakit dengan angka mortalitas dan morbiditas yang sangat tinggi di dunia. Prevalensi hipertensi semakin meningkat setiap tahunnya dan Jawa Barat berada di peringkat keempat dengan prevalensi 29,4%. Penelitian ini bertujuan untuk mengukur tingkat kepatuhan pasien hipertensi terhadap pengobatanya dengan menggunakan kuesioner Eight-Item Morisky Medication Adherence Scale (MMAS-8). Studi ini merupakan penelitian observasional menggunakan rancangan potong lintang, dilakukan pada bulan Oktober 2017–Februari 2018 di fasilitas kesehatan tingkat pertama di Kota Bandung. Sejumlah dua ratus dua puluh enam responden terlibat dalam penelitian ini. Responden mengisi mengisi kuesioner MMAS-8 versi Bahasa Indonesia yang telah divalidasi setelah menandatangani informed consent terlebih dahulu. Hasil penelitian menunjukkan bahwa sebanyak 53,5% dari responden memiliki tingkat kepatuhan rendah, 32,3% dari responden memiliki tingkat kepatuhan sedang, dan 14,2% dari responden memiliki tingkat kepatuhan tinggi. Data kemudian diolah secara statistik menggunakan analisis Chi-Square sehingga diperoleh hasil bahwa terdapat korelasi yang bermakna (p>0,05) antara tingkat kepatuhan terhadap gender, tingkat pendidikan, status pekerjaan, riwayat penyakit keluarga, kejadian komplikasi, dan pengalaman mendapatkan informasi mengenai hipertensi dan pola diet. Terdapat korelasi bermakana antara status tekanan darah (terkontrol dan tidak terkontrol) terhadap kepatuhan responden (p=0,000). Lebih dari 50% pasien hipertensi di fasilitas kesehatan tingkat pertama di Kota Bandung masih memiliki tingkat kepatuhan yang rendah terhadap pengobatanya dengan rate kepatuhan pasien hipertensi terhadap pengobatanya sebesar 26,3%.

Medication Adherence among Hypertensive Patients in Primary Healthcare in Bandung City

Abstract
Hypertension is a disease with high mortality and also mobility all over the world. The prevalence of hypertension is increasing every year and West Java is ranked fourth with a prevalence by 29.4%. This study aimed to measure the level of medication adherence of hypertensive patients by using Eight-Item Morisky Medication Adherence Scale (MMAS-8) questionnaire. This study was an observational study using cross sectional design and conducted in October 2017–February 2018 at primary healthcare facilities in Bandung City. A total of two hundred and twenty-six respondents were involved in the study. Respondents are required to fill out the validated Indonesian version of MMAS-8 questionnaire after signing informed consent. The results showed that 53.5% of the respondents had low level of adherence, 32.3% of the respondents had moderate level of adherence, and 14.2% of the respondents had high level of adherence. Then, data were processed statistically by using Chi-Square analysis and the results showed that there was significant correlation (p>0.05) between the level of adherence to gender, education level, occupational status, family history of disease, incidence of complications, and experience with information about hypertension and diet. There was a significant correlation between the status of blood pressure (controlled and uncontrolled) to respondent’s adherence (p=0.000). In addition, more than 50% hypertensive patients in primary health care in Bandung City still has low level of medications adherence with rate of adherence is 26.3%.
