Extended conization a therapeutic option in cervical carcinoma in-situSe estudiaron 89 pacientes que tenían indicada la histerectomía por portadoras de un carcinoma in situ de cuello uterino en el Servicio de Ginecología del Hospital « Abel Santamaría `' de Pinar del Río, durante los años 1995 y 1996 . Para llegar al diagnóstico definitivo en todos los casos se tomó biopsia de cuello uterino por ponchamiento exclusivamente en 15 casos, por conización habitual en 22 casos y por conización amplia de cuello en la cual el área a incluir a nivel del execervix se extendió más allá de la zona Shiller positivo oo con algún signo sospechoso de lesión al examen clínico en 52 casos. En el análisis de los resultados encontramos que las complicaciones postoperatorias secundarias a la conización amplia del cuello fueron similares a las de la conización habitual; en el estudio histológico del útero post-histerectomía se encontró una disminución en la persistencia de células tumorales a partir de la fecha en que comenzó a realizarse la conización amplia de cuello en nuestro servicio. De las 89 pacientes operadas durante dos años estudiados, en 63 casos no se encontró presencia de células tumorales en el útero analizado post-histerectomía y en 21 de ellas ocurrió alguna complicación postoperatoria; concluimos que la aplicación de la conización amplia de cuello uterino con fines diagnósticos y terapéuticos contribuye a disminuir el gasto hospitalario. Hacemos recomendaciones acerca del seguimiento en la consulta de patología de cuello y por el Médico de la Familia de estas pacientes. ABSTRACT 89 patients were going to be submitted to hysterectomy because of cervix carcinoma in situ in the Service of Gynecology at " Abel Santamaría " General Hospital in Pinar del Río , Cuba, from 1995 to 1995 to 1996. In order to make the definitive diagnosis in all of the cases punch biopsy of the uterine cervix was made exclusively in 15 case by means of habitual conization in 22 cases and by means of wide conization of the uterine cervix in which thearea to be incised at execervix level went beyond the positive Shiller zone or had some suspicious singn of injury at the clinical test in 52 case. In the analysis of results we found that secondary post-operative complications to the wide cone biosy were similar to the habitual conization; in the histological study of the uterus post-histerectomy a decrease in the persistence of tumor cells was found from the date of the beginning of the.
