Strategies for Clinical Decision Support for Electronic Case Reporting

SUMMARY

ObjectiveTo discuss how clinical decision support (CDS) for electronic case reporting (eCR) will evolve over time to provide multiple deployment modelsIntroductionAs the knowledge required to support case reporting evolves from unstructured to more structured and standardized formats, it becomes suitable for electronic clinical decision support (CDS). CDS for case reporting confronts two challenges: a) While EHRs are moving toward local CDS capabilities, it will take several years for EHR systems to consistently support this capability; and b) public health-related CDS knowledge, such as Zika infection detection and reporting rules, may differ from jurisdiction to jurisdiction. Therefore, there is an ongoing need to manage reporting rules in a distributed manner. Similarly, there is a need for more decentralized models of CDS execution to overcome some of the disadvantages of centralized deployment and to leverage local CDS capabilities as they become available over the next several years.MethodsThe Reportable Condition Knowledge Management System (RCKMS) is a project funded by the CDC, through the Council of State and Territorial Epidemiologists (CSTE), to develop a tool that allows jurisdictions to author rules that define whether a patient is reportable for certain conditions. RCKMS includes a Decision Support Service (DSS) that runs the jurisdictions’ rules and determines if a patient is reportable, for which condition(s) and to which jurisdiction(s). RCKMS currently plays a significant role in the broader Digital Bridge project that has been working to provide structure and governance around the national planning and implementation effort of eCR. RCKMS is currently a centralized CDS service that can be accessed by EHRs until they all have local CDS capabilities; and a knowledge authoring environment that allows ongoing distributed rule authoring. RCKMS supports the strategy for public health knowledge management, and it will evolve over time to provide the systems and services to satisfy short-, mid-, and long-term public health CDS requirements. In addition, RCKMS will comply with emerging technical standards that support this work.ResultsRCKMS is currently being deployed as a single, central, national service on the APHL Informatics Messaging Services (AIMS) platform, which is operated and maintained by Association of Public Health Laboratories (APHL). The AIMS platform connects directly with reporters and provides a routing and validation service for incoming and outgoing messages. Two distributed CDS scenarios for decentralized eCR models have been identified. In the first scenario, the Decision Support Service component of the RCKMS software is installed within a clinical organization (as it would be in a centralized service) and executed locally. The second distributed CDS scenario for eCR involves the distribution of the Reporting Specification rules without the software. In this scenario, local electronic health record (EHR) implementations would be required to consume the Reporting Specifications and utilize them in a CDS capability within their EHR.ConclusionsIt is expected that given the diversity of organizations, systems, and architectures in the United States that multiple deployment scenarios for CDS for eCR will be simultaneously deployed for the foreseeable future. It cannot be stressed enough, however, that in all scenarios – centralized and distributed – there must be a centralized and uniform authoring of the Reporting Specification rules, since the specifications themselves originate from public health through a centralized process and must be administered nationally through a well-established process. It is also essential that all sites have all the rules available to them, since there may be multiple jurisdictions whose rules and reporting is required, with the determination of jurisdiction(s) based on where a patient lives and where they receive care.

La evaluación del aprendizaje es una parte esencial del proceso de enseñanza-aprendizaje que nos posibilita su propia dirección, así como el control y la valoración de los modos de actuación que los estudiantes adquieren a través del desarrollo del proceso docente. Las estrategias para el desarrollo de las asignaturas, Propedéutica Clínica y Semiología Médica y de Medicina Interna, están diseñadas de manera tal que permita estimular a los estudiantes en su auto preparación y dotarlos de herramientas y conocimientos básicos en el desarrollo de habilidades necesarias al no contar en el consultorio médico con un registro permanente de las evaluaciones de los estudiantes en estas asignaturas, se decidió por el claustro de profesores de tercer año de la carrera pertenecientes al Policlínico Turcios Lima de Pinar del Río, durante el curso 2008-2009, confeccionar uno en forma de tarjeta que contemplara los principales aspectos a tener en cuenta en el desarrollo de los conocimientos y habilidades obtenidas por los educandos durante su estancia, con el objetivo de controlar de forma sistemática y uniforme la evaluación del estudiante en su desempeño en estas estancias como futuro Médico General Integral. ABSTRACT The evaluation of students' learning development along medical studies has been a dilemma for the medical schools and universities. During many years this evaluation was aimed at measuring knowledge acquisition in each academic discipline. Learning evaluation is an essential part of the teaching-learning process making its direction possible, as well as the control and the assessment of behavior manners that students acquire through the development of the teaching process. The strategies to learn about the development of the subjects: Clinical Propedeutics, Medical Semiology and Internal Medicine are designed to encourage students' self-training and to provide the basic tools and knowledge to develop the necessary skills. A permanent record to evaluate students' development in these subjects does not exist in the doctor's office; thus the teaching staff of the 3rd academic year at "Turcios Lima" Outpatient Teaching Clinic in Pinar del Rio (2008-2009), decided to prepare a card which includes the main aspects to be considered through the development of knowledge and skills during the stay-period with the purpose of controlling systematically and consistently the evaluation of the students' performance during the stay-period as future Comprehensive Doctors. Resulta significativa la transmisión de estos conocimientos durante las clases de Genética Médica para incrementar el acervo cultural de nuestros estudiantes. Conociendo lo difícil que resulta la comprensión de estos temas y la dificultad de no contar con la literatura básica que recoja estos contenidos, se plantea como problema: ¿Cómo contribuir a la preparación científica de los estudiantes sobre los conocimientos de las enfermedades genéticas, su historia y la vinculación con obras de arte? Para dar solución se propone diseñar un cuaderno complementario que contenga la relación existente entre determinadas enfermedades genéticas, su historia y el reflejo de estas en pinturas célebres, para la preparación científica-humanística de los estudiantes. Hacia la confección del trabajo se consideraron: la organizaciónplanificación y el diseño del cuaderno. Cada autor hizo su propuesta individual y después del análisis colectivo y el criterio de los expertos se conformó su estructura final. El cuaderno consta de las descripciones genéticas y clínicas de cuatro enfermedades genéticas, la historia natural de cada una y las estrategias para prevenirlas; así como sus representaciones en pinturas de gran valor artístico. Según los autores, este es un material que favorece la motivación y el interés de los estudiantes por el estudio de la disciplina y por la investigación sobre esta temática, lo cual contribuye a su desarrollo como futuro profesional de la salud.Palabras clave: enfermedades genéticas; obras de arte; cuaderno complementario.ABSTRACT Genetic diseases have been present throughout human evolution and have been recorded in important artistic works. It is significant the transmission of this knowledge during Medical Genetics classes to increase the cultural heritage of our students. Knowing how difficult it is to understand these issues and the difficulty of not having basic literature that collects these contents, the following problem arises: How to contribute to the scientific preparation of students on the knowledge of genetic diseases, their history and the link with works of art? To solve it, it is proposed to design a complementary notebook that contains the relationship between certain genetic diseases, their history and the reflection of these in famous paintings, for the scientific-humanistic preparation of the students. For the preparation of the work they were considered: the organization- Planning and design of the notebook. Each author made his individual proposal and after the collective analysis and the criteria of experts his final structure was conformed. The notebook consists of the genetic and clinical descriptions of four genetic diseases, the natural history of each and the strategies to prevent them; as well as his representations in paintings of great artistic value. According to the authors, this is a material that favors the motivation and interest of the students for the study of the discipline and for research on this subject, which contributes to their development as a future health professional.Keywords: genetic diseases; works of art; complementary notebook.
