Evaluation of adherence to measures for the prevention of surgical site infections by the surgical team

SUMMARY

OBJECTIVEEvaluate pre- and intraoperative practices adopted by medical and nursing teams for the prevention of surgical infections.METHODA prospective study carried out in the period of April to May 2013, in a surgical center of a university hospital in Belo Horizonte, Minas Gerais.RESULTS18 surgeries were followed and 214 surgical gloves were analyzed, of which 23 (10.7%) had postoperative glove perforation detected, with 52.2% being perceived by users. Hair removal was performed on 27.7% of patients in the operating room, with the use of blades in 80% of the cases. Antibiotic prophylaxis was administered to 81.8% of patients up to 60 minutes prior to surgical incision. An average of nine professionals were present during surgery and the surgery room door remained open in 94.4% of the procedures.CONCLUSIONPartial adhesion to the recommended measures was identified, reaffirming a need for greater attention to these critical steps/actions in order to prevent surgical site infection.

Correlación entre la presencia de factores de patogenicidad de Helicobacter pylori y enfermedades digestivas en pacientes con síntomas digestivos. 2012-2016 / Correlation between the presence of pathogenicity factors of Helicobacter pylori and digestive d Correlación entre la presencia de factores de patogenicidad de Helicobacter pylori y enfermedades digestivas en pacientes con síntomas digestivos. 2012-2016 / Correlation between the presence of pathogenicity factors of Helicobacter pylori and digestive dIntroducción: la correlación entre los factores de patogenicidad de Helicobacter pylori y las enfermedades digestivas no ha sido estudiada exhaustivamente en Cuba y no existen reportes de dicha relación en la Atención Primaria de Salud.Objetivo: evaluar la correlación entre algunos factores de patogenicidad de Helicobacter pylori y las enfermedades gastroduodenales en la Atención Primaria de Salud.Métodos: estudio descriptivo en el Policlínico 19 de abril, el Instituto de Medicina Tropical “Pedro Kourí” y el Hospital Clínico Quirúrgico “Manuel Fajardo” desde 2012 hasta 2016 en 60 adultos con síntomas digestivos, a los cuales se les realizo una endoscopia con tres tomas de ponche de biopsia para test de ureasa, histopatología y cultivo de Helicobacter pylori. Se les extrajo sangre venosa para la evaluación de un sistema serológico Western Blot comercial para el diagnóstico de dicha infección. Se utilizó el Chi Cuadrado de Pearson para determinar la posible relación entre los factores de patogenicidad y las enfermedades gastroduodenales. Resultados: Helicobacter pylori fue diagnosticado en 38,3% de los pacientes estudiados y 30% presentaron algún tipo de lesión gástrica. La úlcera péptica y las lesiones gástricas mostraron una asociación estadísticamente significativa con la presencia de proteínas específicas de Helicobacter pylori.Conclusiones: la metaplasia intestinal y la totalidad de las lesiones gástricas se ven de una manera estadísticamente significativa en los pacientes Helicobacter pylori negativos. La proteína CagA predomina en ambos grupos de pacientes. No se observa asociación estadística entre las proteínas específicas de Helicobacter pylori y el status de positividad del microorganismo en los pacientes estudiados. Palabras clave: Serología Western Blot; Helicobacter pylori; Factores de patogenicidad; lesiones gástricasABSTRACTIntroduction: the correlation between the factors of pathogenicity of Helicobacter pylori and digestive diseases has not been studied exhaustively in Cuba and there are no reports of this relationship in Primary Health Care.Objective: to evaluate the correlation between some factors of pathogenicity of Helicobacter pylori and gastroduodenal diseases in Primary Health Care.Methods: descriptive study in the Polyclinic April 19, the Institute of Tropical Medicine “Pedro Kourí” and the Clinical Surgical Hospital “Manuel Fajardo” from 2012 to 2016 in 60 adults with digestive symptoms, who underwent an endoscopy with three biopsy punch samples for urease test, histopathology and culture of Helicobacter pylori. Venous blood was taken for the evaluation of a commercial Western Blot serological system for the diagnosis of this infection. Pearson’s Chi-square was used to determine the possible relationship between pathogenicity factors and gastroduodenal diseases.Results: Helicobacter pylori was diagnosed in 38.3% of the patients studied and 30% presented some type of gastric lesion. Peptic ulcer and gastric lesions showed a statistically significant association with the presence of specific proteins of Helicobacter pylori. Conclusions: the intestinal metaplasia and the totality of the gastric lesions are seen in a statistically significant way in the Helicobacter pylori negative patients. The CagA protein predominates in both groups of patients. There is no statistical association between the specific proteins of Helicobacter pylori and the positive status of the microorganism in the studied patients.Keywords: Western Blot Serology; Helicobacter pylori; Pathogenicity factors; gastric lesions.
